摘要
目的探讨多层螺旋CT直接下肢静脉造影(MSCTA)的临床诊断价值。资料与方法搜集下肢静脉造影21例进行回顾性分析,总结影像学表现,利用评分法估计病变累积范围,同时结合超声进行对比分析。结果21例中正常3例,下肢静脉栓塞18例,其中左侧下肢静脉栓塞11例,右侧下肢静脉栓塞5例,双侧同时出现栓塞2例。正常下肢静脉MSCT造影表现为大隐静脉及股深静脉均显影良好,充盈饱满,管腔内密度均匀,无充盈缺损及狭窄,无侧支循环属支显示,髂外静脉及髂总静脉显示清楚,髂内静脉显示率较低。下肢静脉栓塞MSCT造影表现为血管腔内充盈缺损,管腔变细、变窄或中断,梗阻段以上静脉血管不能显示,深浅静脉交通支明显增多,侧支循环开放,盆腔内静脉丛迂曲扩张。梗阻部位:股静脉16例,髂外静脉1例,髂总静脉1例。梗阻程度评分结果:≤5分12例,5.10分4例,≥10分2例。11例下肢静脉超声检查证实血栓存在与MSCTV完全一致(100%)。结论经下肢静脉注射对比剂,能避免动脉伪影的干扰且可直接显像,多种重建技术可明确病灶的范围和程度,对侧支循环及交通支的精确显示是MSCTV的优点。
Objective To discuss the clinical value of direct multi-slices CT venography in lower extremity vein. Materials and Methods 21 eases with direct muti slices CT venography in lower extremity vein were reviewed retrospectively, and their characteristic features were summarized. The lesion scope was estimated with score, results were compared to US doppler. Results Among 21 cases, there were 3 cases in normal, thrombosis of lower extremity vein in 18 cases (left inll, right in 5, both in 2 cases ). The normal lower extremity vein in MSCTV was the saphenous vein and deep femoral vein imaging well, which were filling full with density uniformity in vessel, and without filling defect, stenosis, and collateral circulation. External lilac vein and common iliac vein were clearly depicted, hut internal iliac vein was not displayed clearly. MSCTV findings of the deep vein thrombosis were filling defect in vessel and thinning, narrowing and occlusion in the vessel, and the vessel of upper obstruct cannot display, and communicating branch and collateral circulation were redundant, and the venous plexus in pelvic cavity was distorted and expand.. Obstructive location: 16 cases in femoral vein, and 1 case in external iliac vein, and 1 case in common lilac vein. Obstruct extent score result: 12 cases were ≤5 score and 4 cases 5 - 10 score, and 2 cases≥ 10 score. Results of 11 US examinations were coincident with MSCTV. Conclusion Direct multi slices CT Venography of lower extremity may avoid to interfere with artifact of artery, multi reconstruction techniques may identify the content and scope of lesion, MSCTV has the advantage to depict clearly for collateral circulation.
