颈动脉压迫训练中SPECT对大脑缺血的耐受性的判断 被引量:3

摘要 目的对SPECT在颈动脉压迫训练中对大脑耐受性的判断做出评价。方法21只狗分为4组,直接结扎组6只,植入式训练组6只,自动监控式颈动脉压迫训练组6只,对照组3只。前3组狗使其动脉血压降低30~40mmHg,直接结扎组结扎其右颈总动脉;植入式训练组逐步阻断右颈总动脉血流,直到全部阻断进行植入式压迫训练;体外压迫训练组利用压迫训练器行压迫训练,1次/日,开始为压迫30分钟,以后每天延长10分钟,直至第8天处死。对照组直接结扎其颈动脉。前3组狗均做SPECT检查,4组狗均做病理检查。结果对照组无明显异常;两训练组狗SPECT值与直接结扎组相比得到了显著提高;三组狗SPECT实验值与其自身正常值相比,差异显著。SPECT值与病理检查结果相吻合。结论SPECT对于判断颈动脉压迫训练时,大脑两侧半球侧支循环的建立,效果显著。 Purpose To evaluate the judgement value of SPECT in determination of the Brain’s torlerance of cerebral ischemia in carotid artery(CA) compression. Methods 21 dogs were devided into four groups:hypotension with directly right carotid ligation group (HDL) 6 dogs,hypotension with implantable CA occluded training group (HIT) 6 dogs,hypotension with CA compression training group (HCT) 6 dogs,control group 3 dogs.The first three groups were inducted hypotension,the HDL dogs were ligated their right CA,the HIT dogs right CA blood flow was gradualy reduced till the flow was utterly occluded;the HCT dogs applied the CA compression training once a day,which began with 30 minuetes first day and prolonged 10 minutes more next day untill they were killed on the 8 th day.the HDL,HIT HCT dogs were examined with SPECT,all dogs brain tissues were observed with pathological method.Results The HIT and HCT dogs’ SPECT results were significantly improved compared with HDL group,all these three group’s SPECT results have remarkably diffrence with the result of their mode with only hypotension.The SPECT result accords with pathological result.Conclusion SPECT can act important role when judging the brain’s torlerance of the cerebral ischemia in two occluded training methods.It can predict the forming of the tributary circulation of brain’s both sides. [
