摘要
目的:评价顺义区现行乙肝疫苗(HepB)免疫策略的母婴阻断效果。方法:利用顺义区2005年-2009年对母亲乙肝病毒表面抗原(HBsAg)阳性的儿童进行HepB免疫后的血清学监测资料,分析其血清HBsAg、乙肝病毒表面抗体(抗-HBs)和乙肝病毒核心杭体(抗-HBc)水平以及疫苗保护水平。结果:所监测的287名儿童中,HBV感染率为2.79%,HepB保护率为96.72%。母亲双阳性者儿童HBV感染率显著高于母亲单阳性者。抗-HBs阳性率为83.97%,各年份抗-HBs阳性率差异具有统计学意义;HepB首针及时的儿童抗-HBs阳性率明显高于首针不及时的儿童。抗体GMC水平为138.44 mIU/ml,2007、2008和2009年抗体GMC水平明显高于2005和2006年;HepB首针及时的儿童抗体GMC水平明显高于不及时的儿童。采用HBIG联合免疫的儿童抗体GMC水平明显高于未使用者。结论:顺义区现行HepB免疫策略对母亲HBsAg阳性儿童的免疫保护效果比较理想。
Objective:To evaluate the efficacy of Hepatitis B vaccine in the children born to HBsAg carrier mothers under the Hepatitis B vaccine immunity strategy of Beijing Shunyi District.Methods: The supervising blood serum data of the children born to HBsAg carrier mothers in the years from 2005 to 2009 were collected.The positive rate of HBsAg,anti-HBs,anti-HBc and the protective rate were analyzed.Results: A total of 287 children born to HBsAg carrier mothers were vaccinated according to the Hepatitis B vaccine immunity strategy of Beijing.The infective rate of HBV and protective rate of HepB were 2.79% and 96.72% respectively.The infective rate of HBV of the children born to HBsAg and HBeAg carrier mothers was higher than the rate of the children born to HBsAg carrier mothers.The positive rate of anti-HBs was 83.97%.There were statistics significance in different years.The anti-HBs positive rate of the children who first needle were timely was higher than the rate of the children who first needle were no timely.The GMC of anti-HBs was 138.44 mIU per mL.The anti-HBs GMC of 2007,2008 and 2009 were higher than those of 2005 and 2006.The anti-HBs GMC of the children who the first needle was timely was higher than that of the children who the first needle was no timely.The anti-HBs GMC of the children who HBIG were adopted was higher than that of the children who HBIG weren′t adopted.Conclusion: The Hepatitis B vaccine immunity strategy of Shun Yi District had good immunological and protective efficacy to HBV interruption of perinatal transmission.
