摘要
目的:观察实验兔急性肺损伤中血管内皮标记物的变化。方法:采用同种骨髓提取液(BME)经兔颈静脉缓慢恒流注入致伤的方法,复制兔骨髓型急性肺损伤(ALI)模型,连续6h多时相点观察血中循环内皮细胞(CEC)、血浆颗粒膜蛋白-140(GMP-140)和内皮素(ET-1)以及肺组织GMP-140含量,并行常规病理及电镜检查。结果:致伤后0.5h,CEC、血浆GMP-140和ET-1均显著升高,持续6h仍显著高于伤前和对照;其中,血浆GMP-140升高达3.25倍,1h时达到高峰,而肺组织GMP-140含量降低。血浆GMP-140早期升高,幅度较大,与PaO2负相关,与其它指标亦有较好的相关性。结论:经兔颈静脉缓慢恒流速注射BME(0.35ml/kg,2ml/h)能复制出较理想的ALI模型。检测血浆GMP-140可能为临床ALI的早期诊断、病情监测和评估提供依据。
Objective: To study the changes of the markers of pulmonary vascular endothelial
cells (PVEC) in acute lung injury (ALI) in rabbits. Methods: 31 rabbits were randomized into the
control group ( n =10) and the bone marrow extract group ( n =21, BME). The animals of BME
group received an injection of 0.35 ml/kg of BME in an injection speed of 2 ml/hour. All rabbits
were killed 6 hours later. Granular membrane protein 140 (GMP 140), endothelin 1 (ET 1) and
circulating endothelial cells (CECs) were measured with immunoradiometric assay (IRMA),
radioimmunoassay (RIA), and immunohistochemistry (IHC) respectively. Results: GMP 140, ET
1 and CEC began to increased in the first 0.5 hour and the increase lasted for 6 hours. The
drammatical elevation of plasma level of GMP 140 (3.25 times) was negatively correlated to
PaO 2 but positively correlated to other parameters. The GMP 140 on the surface of PVECs
was weakly revealed with IHC staining. Conclusion: Slow rate injection of BME can establish
and acceptable model of ALI and the determination of GMP 140 might beaa satisfactory
criterion for early surveillance and for the prognosis of ALI in clinical practice.
