摘要
目的:总结国内外对前哨淋巴结在结直肠癌领域的研究现状。方法:应用PubMed及CNKI期刊全文数据库检索系统,以"前哨淋巴结、结肠癌和直肠癌"等为关键词,检索1977-01-2009-06的相关文献,共检索到英文文献156条,中文文献62条。纳入标准:1)前哨淋巴结的起源及特点;2)前哨淋巴结微转移的研究;3)前哨淋巴结活检技术的应用研究;4)前哨淋巴结在结直肠癌领域的应用发展。根据纳入标准,纳入分析28篇文献。结果:前哨淋巴结状态可以比较准确地预测结直肠癌区域淋巴结的转移情况,对前哨淋巴结行集中分析有助于发现传统组织病理学检查遗漏的微转移灶及异常淋巴引流途径,从而使部分患者肿瘤分期上调。结论:结直肠癌前哨淋巴结活检是一种切实可行的技术,有望为临床判断区域淋巴结群受累状况提供一种新的手段,从而有助于更准确的肿瘤分期。但仍需进一步大样本前瞻性的研究。
OBJECTIVE：To summarize the research of sentinel lymph node（SLN） in colorectal cancer home an abroad.METHODS：By PubMed and CNKI retrieval system,with ＂sentinel lymph node,colonic cancer and rectal cancer et al.＂ as the key words,156 papers of literature in English and 62 in Chinese were retrieved from Jan 1977 to Jun 2009.The selection standards for the data：1）the origin and the characteristics of the SLN;2）the research of micrometastasis of the SLN;3）the application of the sentinel lymph node biopsy（SLNB）;4）the progress in the SLN in colorectal cancer.According to the above standards,28 were finally analyzed.RESULTS：The results of SLNB can accurately reflect the status of the regional lymph nodes in colorectal cancer.Focused examination of the SLNs can identify micrometastases and aberrant lymphatic drainage that might otherwise have been missed by routine histopathological analysis,thus upstaged this group of patients.CONCLUSION：SLNB represents a new and effective technique to predict the tumor status of regional lymph nodes,which offers a potential alternative to improve the accuracy of tumor staging in colorectal cancer.However,further research is required.
