摘要
【目的】探讨Survivin和p16INK4a在宫颈癌(CC)及其癌前病变中的表达及二者的相关性。【方法】采用免疫组化SP方法检测Survivin和p16INK4a在50例宫颈上皮内瘤变(CIN)50例CC和10例正常宫颈组织标本中的表达。【结果]Survivin在正常宫颈、宫颈CIN和UCC中的总阳性率分别为0%、50%和76%(X2=21.651,P〈0.05),其中,宫颈CIN和CC标本的阳性率均高于正常宫颈标本(X2=8.571,X2=20.727,P〈0.0125),CC组的Survivin阳性率亦高于宫颈CIN(X2=7.250,P〈0.0125)。p16INK4a“在正常宫颈、宫颈CIN和CC中的总阳性率分别为0%、62.0%和84.0%(X2=27.123,P〈0.05),其中,宫颈CIN和CC标本的阳性率均高于正常宫颈标本(x2=12.828,x2=28.000,P〈0.0125),CC组的p16INK4a“阳性率亦高于宫颈CIN,但差异无显著性(x2=6.139,P〉0.0125)。p16INK4a及Survivin在CINI、CINII中的表达均呈负相关,但CC中表达呈正相关(P〈0.01)。p16INK4a和Survivin表达的阳性率与CC病理学分级、临床分期及淋巴结转移相关(P〈0.05)。【结论]Survivin和p16INK4a阳性表达与CC及其癌前病变密切相关,联合检测宫颈病变中Survivin和p16INK4a的表达有助于CC的早期检出和预后预测。
[Objeetive]To evaluate the clinical significance of survivin and p16INK4a expression in uterine cervix cancer(UCC) and cervical intraepithelial neoplasia(CIN). [Methods] The expression of survivin and p16INK4a was detected in 50 CIN cases, 50 UCC cases and 10 normal cervix cases by immunohistochemical staining SP method. [Results] The positive rate of survivin in normal cervixes, CINs and UCCs was 0, 50.0% and 76.0%, respectively(x2 =21. 651, P 〈0.05), and the positive rate of survivin in CINs and UCCs was higher than that in normal cervixes(x2= 8. 571, ;x2 =20. 727, P 〈0. 0125), and the positive rate in UCCs was higher than that in CINs(x2 =7. 250, P 〈0. 0125). The positive rate of p16INK4a in normal cervixes, CINs and UCCs was 0, 62.0% and 84.0%(X2=27. 123, P d0.05), and the positive rate of p16INK4" in CINs and UCCs was higher than that in normal cervixes(y1 2 = 12. 828,X2 2 =28. 000, P 〈0. 0125), while the positive rate in UCCs was higher than in CINs without statistical significance(xz = 6. 139, P 〉0. 0125). Survivin and p16lNK4a showed negative correlation in CIN I and CIN , but positive correlation in UCC( P 〈0.01). Sur- vivin and pt61INK4a was related with pathological grade, clinical stage and lymph node metastases of UCC( P 〈 0.05). [Conclusion]Survivin and p16INK4a expression is related closely to UCC and CIN, and the detection of survivin and p16INK4a in cervix diseases will be helpful for early detection and prognosis prediction of UCC.
