应用瞬时波强技术对扩张型心肌病左心室功能的临床研究 被引量:3

Evaluation of left ventricular function in patients with dilated cardiomyopathy using wave intensity

摘要 目的探讨瞬时波强(WI)技术评价扩张型心肌病(DCM)患者左心室功能的价值。方法选取DCM患者27例,对照组50例。常规超声心动图测量左心大小、左室射血分数(LVEF);TVI测量二尖瓣环水平舒张早期速度(Ve)及心房收缩期速度(Va);应用WI评估DCM患者颈总动脉W1、W2、R—W1和W1-W2等指标,与对照组进行对比分析。结果DCM组LVEF、Ve及Va较对照组明显降低(P〈0.01)。DCM患者二尖瓣平均反流面积(7.34±2.37)cm^2。W1较对照组明显降低(P〈0.05),W2与对照组比较差异无统计学意义(P〉0.05),W1-W2、W1-W2correct明显缩短(P〈0.05),R—W1、R-W1correct明显延长(P〈0.01)。W1与LVEF相关系数为0.72(P〈0.01)。结论WI技术有助于评估DCM左室收缩与舒张早期功能改变。 Objective To explore the clinical value of wave intensity(WI) in evaluating the left ventrieular function in patients with dilated cardiomyopathy(DCM). Methods Twenty-seven DCM patients and fifty healthy volunteers were involved in the study as the case and control group seperately. Diameter of left atrium and left ventricle,left ventricular ejection fraction(LVEF) were measured by conventional echocardiography. Mean value of the diastolic myocardial velocity(Ve, Va) of the six region of intrest (ROI) at the mitral annulus level were detected by tissue velocity imaging. WI indices including W1 , W2 , R-W1 , R-W1correct, W1 -W2 and W1 -W2correct were measured. Differenees of the parameters between the groups were analyzed. Results LVEF, Ve and Va in case group were significantly lower than those of the control group ( P 〈 0.01). Area of mitral regurgitation were detected in all of the 27 DCM patients with a mean value of (7.34 ± 2.37)cm^2. Compared with the control group, W1 ,W1-W2 ,W1-W2 in case group were decreased significantly ( P 〈0.05 or P 〈0.01),and R-W1 ,R-W1 were prolonged significantly( P 〈0.01). However, no significant were got between the comparition of W2 of the two groups( P 〉0.05). The correlation coefficient of W1 and LVEF was 0.72( P〈0.01). Conclusions WI was a useful technique to evaluate the left ventricular function in patients with DCM.
