摘要
目的:观察组织蛋白酶D在结直肠癌患者的癌组织及正常肠壁中的表达及其在两种组织中的定位,探讨结直肠癌中组织蛋白酶D的表达与临床病理的关系。方法:取30例结直肠癌患者的癌组织和正常肠壁组织,用免疫荧光流式细胞术检测组织蛋白酶D在两种组织内的表达量,用免疫组化染色观察这种酶在两种组织中的定位。分析结直肠癌中组织蛋白酶D表达与临床病理参数的关系。结果:组织蛋白酶D在肿瘤组织表达量高于正常组织(P<0.01),在两种组织内的表达量无相关性(P>0.05)。不同Dukes分期中大肠癌组织蛋白酶D表达量差异有统计学意义(P<0.05),其分期越晚,表达量越高;癌组织沿肠壁浸润范围越大,该酶表达越强(P<0.05);患者性别、年龄、肿瘤形态、分化程度之间组织蛋白酶D的表达差异无统计学意义(P>0.05)。免疫组化方法观察到组织蛋白酶D阳性表达的染色颗粒主要位于癌细胞及间质细胞的胞浆近包膜处,正常肠壁的黏膜上皮细胞内阳性颗粒多聚集在细胞核周围。结论:结直肠癌患者的癌组织中组织蛋白酶D表达明显高于正常肠壁组织,在两种组织细胞中的分布形态也不同,随着Dukes分期和浸润范围表达而增强,与结直肠癌浸润、转移相关。
Objective：The aim of this study was to evaluate the expression of cathepsin D in colorectal cancer and normal colorectal tissue,to investigate the correlations with clinicopathological parameters.Methods：Tumor and normal specimens from 30 colorectal cancer patients were used in the study,the expression of cathepsin D in those tissue was detected by flowcytometry and immunoflurescence staining,the location of cathepsin D in tissue was observed by immunohistochemistry.Result：The expression of cathepsin D in normal colorectal tissue was lower than that in colorectal cancer（P0.05）,there was no significant correlation with the expression of cathepsin D between cancer and normal tissue（P0.05）.The expression of cathepsin D in tissue of colorectal cancer was significantly higher in Dukes C stage than in Dukes A or B stage（P0.05）.Furthermore,with the extension of tumor invaded along colorectal bowel wall,the cathepsin D expressed higher（P0.05）.The expression of cathepsin D was not significantly correlated with gender,age and histological（P0.05）.Immunostaining with cathepsin D was predominately localized in cytoplasm of tumor and stromal cell,the immunoreactive granular localized on the basal portion of the cytoplasm.The healthy colorectal epithelial cell exhibitde that immunoreactive granular localized on the supranulcear portion of the cytoplasm.Conclusion： The expression of cathepsin D in tissue of colorectal cancer was significantly higher in normal tissue,localization of cathepsin D in cancer cell was not alike in normal epithelial cell.There were the correlations with the expression of cathepsin D and Dukes stage of colorectal cancer or the degree of invasion,cathepsin D was implicated in the process of colorectal cancer invasion and metastasis.
