Comparison between nasal continuous positive airway pressure ventilation and conventional mechanical ventilation in treatment for neonatal respiratory distress syndrome

摘要目的观察新型鼻塞持续正压通气(NCPAP)与常规机械通气(CMV)对新生儿呼吸窘迫综合征(NRDS)的治疗效果和NCPAP是否降低呼吸机相关性肺炎(VAP)、呼吸机所致肺损伤(VILI)等发生。方法将2004年9月-2008年3月42例确诊为NRDS的患儿随机分为两组,一组为CMV组,另一组为NCPAP组,均应用肺表面活性物质(猪肺磷脂)。CMV组应用美国产VIP-BIRD呼吸机(IPPV+PEEP),NCPAP组应用英国EME公司生产的Infant FlowTM System新型鼻塞CPAP仪(NCPAP)。观察比较两种方法治疗NRDS的疗效情况。结果NCPAP组存活20/21例,CMV组18/21例,两组在存活率、用机时间、颅内出血上差异无统计学意义;NCPAP组发生VAP、VILI并发症明显低于CMV组(χ2=17.53、4.86,P均<0.05)。结论新型NCPAP及肺表面活性物质替代治疗NRDS是有效的,具有相对无创、简便易行的优点,并可减少VAP和VILI的发生。
