肠道屏障在多器官功能不全综合征中的发病学意义探讨 被引量:26

Damage of gut barrier in the pathogenesis of multiple organ dysfunction syndrome

摘要 目的探讨肠道屏障的损伤机制和肠道屏障在多器官功能不全综合征(MODS)中的发病学意义。方法本实验采用酵母多糖对大鼠行腹腔注射制备 MODS 模型。实验用 SD 大鼠24只,随机分为模型组和对照组。酵母多糖剂量为1.0mg/g 体重。结果 MODS 时外周静脉血和门静脉血中内毒素水平明显增高(P<0.01);血和小肠组织中肿瘤坏死因子α(TNFα)含量升高(P<0.05);血液中二胺氧化酶(DAO)含量明显升高(P<0.01),肠粘膜组织中 DAO 含量明显下降(P<0.01)MODS 时回肠粘膜出现明显病理改变,肠粘膜通透性和肠细胞渗透性明显增高。结论 MODS 时存在着严重的肠道屏障功能损伤,肠道屏障功能衰竭是导致 MODS 的重要环节,肠源性内毒素是 MODS 的重要致病因子,组织过氧化损伤是肠道屏障损伤的重要原因之一。保护肠道屏障,减少肠源性内毒素的产生和吸收,防止过氧化损伤将成为防治 MODS 的重要措施。 Objective This study was to determin the relationship between multiple organ dysfunction syn- drome(MODS)and gut barrier failure and the effect of endotoxin derived from the gut.Methods 24 SD rats were divided into two groups,model group(n=12)and control group(n=12).The experimental rats were sacrificed 48h after intraperitoneally injection of zymosan at a dosage of 1.0mg/g body weight.Results The levels of serum endotoxin in the peripheral and portal blood were markedly ele- vated(P<0.01),the DAO concentrations decreased in the ileum tissue(P<0.05),but increased in the blood(P<0.01),the levels of XOD,MDA and TNF in the blood and ileum tissue significantly increased (P<0.01),the levels of GSH decreased(P<0,01).The histopathologic lesions were found in the intes- tine of the model group,including acute necrosis of intestinal mucosa,granulocyte infiltration,and in- creased permeability of intestinal mucosa and enterocytes.The multiple linear regression analysis showed that there were significant relationships among endotoxin,DAO,GSH,XOD,MDA and TNF. Conclusions The results demonstrated that there was a severe gut barrier damage while MODS devel- oped,gut barrier failure can lead to MODS.Endotoxin derived from the gut was the pathogenic factor of MODS and injury of the intestine caused by peroxidase can result in gut barrier failure and MODS.
