摘要
目的研究口腔鳞癌组织中p27Kip1及细胞周期蛋白D1(cyclinD1)表达以及与临床生物学特性和细胞增殖的关系。方法采用免疫组化方法检测53例原发性口腔鳞癌组织及20例正常口腔黏膜组织中p27Kip1及cyclinD1蛋白的表达水平,Ki-67作为细胞增殖活性的指标也被检测。结果p27Kip1在口腔鳞癌中阳性表达率显著低于正常口腔黏膜组织(P<0.05),cyclinD1蛋白和Ki-67在口腔鳞癌中阳性表达率显著高于正常口腔黏膜组织(P<0.05);cyclinD1蛋白过表达与临床分级及淋巴结转移有关(P<0.05),p27Kip1缺失表达与淋巴结转移有关(P<0.05);p27Kip1负表达和cyclinD1过度表达与细胞增殖评价因子Ki-67呈负相关(r=-0.587和-0.667,P<0.05)。结论在鳞癌的发生发展过程中,p27Kip1的缺失表达和cyclinD1的过度表达两者可能单独或共同参与。p27Kip1的缺失可能还与口腔鳞癌细胞的分化、转移及增值和预后有关。
Objective To study the expressions of p27^Kip1 and cyclin D1 proteins in oral squamous cell carcinoma（OSCCs） and their clinical significance.Methods Immunohistochemical technology （P-V） was applied to detect the expression of Cydin D1 and p27^Kip1 in 53 cases of oral squamous cell carcinoma and 20 cases of normal oral mucous membrane. 31ae expression of Ki-67 labeling index（LI）was also examined in order to evaluate cell proliferation activity. Raesults （1） The expression of p27^Kip1 in the tumorous cell was significantly lower than that in normal epithelium （P 〈0.05） ,the expressions of cyelin D1 proteins and Ki-67 in the tumor cells were significantly higher than that in normal epithelium （P 〈0.05）. （2） The positive rate of eyclin D1 was significantly correlated with clinical stage and lymph node metastases （P 〈0.05） ,the positive rate of p27^Kip1 was significantly correlated with lymph node metastases （P 〈0.05）. （3） There was correlation between the expression of cyclin D1 and the expression of p27^Kip1 （r = -0.587,P 〈0. 05）,there was correlation between the expression of Ki-67 and the expression p27^Kip1 （r= -0. 667,P 〈0.05）. Conclusion In the course of pathogenesis and development of oral Squamous cell carcinoma,loss of p27^Kip1 expression and overexpression of cyclin D1 may be individual or conjunct enlist, loss of p27^Kip1 expression may also relate to the differentiation and metastasis of the tumor.
