快速导流杂交法检测人乳头瘤病毒基因分型及其临床意义 被引量:19

摘要 目的评价快速导流杂交法检测人乳头瘤病毒基因分型的临床应用效果,并探讨其临床意义。方法对2006年5月~2007年4月中山大学第二附属医院妇产科及体检中心送检的1022例样本全部进行快速导流杂交法HPV基因分型检测,并将其中563例与细胞学诊断结果做对比。结果1022例样本中,HPV感染率为37.57%(384/1022),广州地区最常见的HPV基因型依次为:16、52、58、68、CP8304、18、33、53、31、11、39、66。随着宫颈病变程度加重,高危型HPV感染率逐渐增高,CINⅠ为32.40%(35/108),CINⅡ为56,67%(34/60),CINⅢ为78,05%(32/41),宫颈癌为95.23%(40/42),与正常体检组(10,53%,16/152)相比,均有显著性差异(P〈0.05)。快速导流杂交法诊断宫颈癌的敏感性为95.23%(40/42),特异性为66.99%(349/521),阳性预测值为18.86%(40/212),阴性预测值为99.43%(349/351)。结论快速导流杂交法可准确进行HPV基因分型,诊断宫颈癌快速敏感,有较高的阴性预测值,是理想的宫颈癌粗筛方法,HPV基因分型也为本地区HPV的流行状况及基因疫苗研究提供了实验依据。 Objective To analyze tile results of gnetyping of high risk papillomavirus and its clinical significance. Methods The genotypes of 1 022 specimen of cervical secretion from the Second Affiliated Hospital of Sat - Yensen University form May 2006 to April 2007 were detected with HybriMax and 563 out of the 1 022 samples were compared with that of histological diagnosis simultaneously, Results Among 1 022 specimen, the infection rate of HPV was 37,57%. The 10 most common HPV genotypes (in descending sequence) in Guangzhou were in the order of 16, 52, 58,68, CP8304, 18, 33, 53, 31, 11 . The more serious the cervical diseases was, the higher positive rates of high - risk types of HPV infection would be. The infection of high risk HPV at histological phase of CIN Ⅰ , CIN Ⅱ, CINⅢ and cervical cancer were 32.40% (35/108),56.67% (34/60),78.05% (32/41) and 95,23%(40/42), respectively, all higher than that of the normal group (10.53%, 16/152) (P 〈 0. 05), The sensitivity, specificity, positive and negative predictive values of the HybriMax in diagnosis of cervical cancer were 95,23%, 66.99%, 18. 86 % and 99,43 % , respectively, Conclusion The HybriMax is a rapid, sensitive and accurate method for genotyping of HPV and it can also be used for screening cervical canee and thus it is also helpful for screening of genetic vaccine,
