摘要
目的:分析人乳头状瘤病毒(human papillomavirus,HPV)在女性人群中的感染状况及其分布规律,探讨HPV感染与宫颈病变发生的关系。方法:对908例宫颈细胞样本,采用凯普医用核酸分子快速杂交基因分型试剂盒(HybriMax)作HPV基因分型检测,并选择其中同期有病理切片结果的妇科住院患者131例,按病理结果分成5组:慢性炎症组、宫颈上皮内瘤变(cervical intraepithelial neoplasia,CIN)Ⅰ组、CINⅡ组、CINⅢ组、宫颈癌组,分析浙江地区HPV感染状况以及HPV基因型在各组疾病中的分布。结果:908例样本中,共检测到21种基因型中的18种,未发现35、43、59 3种亚型。HPV总感染率为30.3%(275?908),其中≤35岁女性的感染率为40.2%(119/296),随着年龄增大感染率下降,55岁以上女性HPV感染率又升高。宫颈病变越严重,HPV感染率越高,慢性炎症组HPV阳性率为40.7%(22/54),CINⅠ组为49.8%(11/23),CINⅡ组为64.0%(16/25),CINⅢ组为78.9%(15/19),宫颈癌组为90.0%(9/10),与正常体检组13.1%(21?160)比较,均存在统计学差异(P<0.05)。各亚型在不同级别宫颈病变中分布不同,致病力有差异,细胞学正常组最常见的HPV基因型为52、58、16、18、39,细胞学异常组最常见的HPV基因型为16、52、58、18、39,宫颈癌中主要感染基因型为16型(70.0%)。结论:HPV感染与宫颈病变关系密切;16型感染是宫颈癌的最大威胁;HPV-DNA分型检测对于宫颈癌高危人群的筛选、临床诊断和预后判断有重要意义。
Objective:To investigate the distribution character of human papillomavirus(HPV) infection in the female and explore the risk of cervical diseases associated with HPV genotype.Methods:A total of 908 cervical cell samples were used to detect HPV genotypes with HybriMax.Among them 131 women who were gynecological inpatients were choosed,they were classified into 5 groups according to their pathologic examination: chronic cervicitis group,CINⅠgroup,CINⅡ group,CINⅢ group and cervical cancer group.The distribution data of HPV genotype was statistically analyzed in Zhejiang Province.Results:Eighteen genotypes were found by HybriMax in 21 HPV genotypes,except HPV35,HPV43 and HPV59.The HPV infection rate among the 908 cases is 30.3%(275∕908),among which,the infection rate of ≤ 35-year-old female is 40.2%(119/296),as the age increases,the infection rate decreases,but HPV infection rate increases in women over the age of 55.The more serious the cervical diseases were,the higher positive rates of HPV infection would be.The infection rates of HPV at histological phase of chronic cervicitis,CINⅠ,CINⅡ,CINⅢ and cervical cancer group were 40.7%(22/54),49.8%(11/23),64.0%(16/25),78.9%(15/19),90.0%(9/10),respectively,all higher than that of the normal group 13.1%(21/160)(P0.05).The distribution of the HPV genotype is differently distributed in various histologic-grade diseases,HPV52,58,16,18,39 were common in normal group,and HPV16,52,58,18,39 were common in abnormal group.The most common genotype was HPV16(70.0%) in cervical cancer group.Conclusion:HPV infection is closely related to cervical diseases.HPV16 infection is the most common genotype in cervical cancer.HPV-DNA genotyping is of great importance for high-risk group screening of cervical cancer,clinical diagnosis and prognosis.
