原位杂交与免疫组化方法在子宫颈病变中检测HPV效果对比分析 被引量:2

CONTRAST ANALYSIS OF THE HPV DETECTION EFFECT OF ISH AND IHC IN CERVIX DISEASES

摘要 [目的]探讨在子宫颈活检组织中检测人乳头瘤病毒(human papillomavirus,HPV)较敏感和准确的技术方法。[方法]采用一组子宫颈鳞状细胞癌(cervical squamous cell carcinoma,CSCC)和子宫颈上皮内瘤变(cervical intraepithelial neoplasia,CIN)活检组织病例,分别做原位杂交(In situ hybridization,ISH)技术检测HPV-DNA和免疫组化(immunohistochemical,IHC)技术检测HPV-Ag。在光镜下对比观察,统计分析。[结果]30例CSCC和33例CIN,用ISH方法阳性率分别为56.7%和69.7%;用IHC方法阳性率分别为0.0%和15.2%,ISH与IHC对比,阳性率显著增高(P﹤0.01)。5例IHC的阳性病例,ISH检测全部为阳性;IHC阴性的病例,ISH检测64.3%阳性,在连续切片上显示,ISH比IHC的阳性细胞数明显增多,且强度增加。[结论]用ISH技术检测HPV特异性好,灵敏度高,明显优于IHC方法,提倡在妇科临床病理工作中放弃IHC,应用ISH检测技术,有益于HPV的检出与防治。 [Objective] To explore a more sensitive and accurate technical method for examining the human papilloma virus (HPV) in biopsy of cervical squamous cell carcinoma (CSCC) and cervical intraepithelial neoplasia (CIN). [ Methods HPV were detected with In situ hybridization (ISH) and immunohistochemical (IHC) staining (S-P) in series biopsy of CSCC and CIN, and were studied contrastively by light microscope and statistical analysis. [ Results] In 30 cases of CSCC and 33 eases of CIN, the positive rates of HPV-DNA were 56.7% and 69.7% under ISH , whereas 0.0% and 15.2% ander IHC respectively. There were significant differences between ISH and IHC (P 〈 0.01). 5 IHC positive cases were all positive by ISH, and in the 28 IHC negative cases, 18 cases (64.3%) were positive by ISH .In the serial sections, ISH positive cells were more than IHC and the intensity increased. [Conclusion] The specificity and sensitivity of HPV examination ate both higher by the ISH technology, which is obviously surpasses the IHC method. We suggest giving up IHC and using ISH in gynecology and clinicalpathology.
