摘要
目的:应用实时三维超声心动图(RT-3DE)对心肌致密化不全(NVM)左心室几何形态进行分析。方法:选取11例NVM患者,应用RT-3DE技术对其左室腔的几何学进行三维图像分析,并测量心尖非致密层厚度(NC)与致密层厚度(C),计算NC/C值。结果:RT-3DE技术能够全面地观察局部心肌的立体解剖结构,清晰显示肌小梁及其深隐窝,分辨出NVM左室的蜂窝状几何学特征;NVM组的NC/C值较对照组明显增大(2.78±0.33对0.98±0.26,P〈0.001);RT-3DE技术有助于分析NVM受累节段的分布范围及节段运动异常情况。结论:RT-3DE技术能全面了解非致密层心肌节段的空间分布状况,是观察NVM形态学改变的最佳影像学手段之一。
Objective To assess the changes of left ventricular geometry in patients with noncompaction of the ventricular myocardium(NVM) by real-time three-dimensional echocardiography(RT- 3DE). Methods Eleven patients with NVM were analyzed with RT-3DE. Geometric appearance of left ventricle was defined and the ratio of both myocardial layers with noncompaction and compaction(NC/C) was calculated. Results NVM was characterized by polypoid endocardial trabeculations,highly trabeculated myocardium,deep recesses. Typical appearance of ventricular myocardium and the honeycombed-appearing structure were observed by RT-3DE. NC/C in NVM group was higher than that in control (2.78± 0.33 vs 0.98± 0.26, P 〈 0.001 ). The ability of RT-3DE in mapping distribution of both segments with noncompaction or hypokinesis would be really useful in analysis of the degree of NVM. Conclusions The spatial distribution of the segments of noncompaction within left ventricle could be described by RT-3DE, which is one of the best imaging methods in assessment of NVM.
