实时三维超声智能容积断层扫描技术定量分析左心室致密化不全 被引量:1

Quantitative analysis of left ventricular noncompaction by iSLICE technology of real-time three-dimensional echocardiography

摘要 目的应用智能容积断层扫描(iSLICE)技术对左心室非致密化组织进行定量分析。方法选择左室心肌致密化不全(NVM)组与对照组各14例,应用飞利浦iE33型彩色超声仪,进行全容积实时三维超声图像采集。使用iSLICE技术,采用自制的面积测量软件,从心尖段切面至二尖瓣瓣尖部水平切面进行分层,测量、计算每个iSLICE图像上的非致密化组织面积比值(ratio of noncompaction area,RNCA)。结果iSLICE图像观察到NVM患者左室腔内非致密结构分布较广。NVM患者的心尖部布满非致密化组织(RNCA达100%),从乳头肌下段水平切面至二尖瓣瓣尖部水平切面的RNCA值为62.14%~40.00%,与对照组比较差异有统计学意义(P〈0.001)。结论iSLICE技术为临床准确判定NVM提供了新的定量观察手段。 Objective To analyze structure of noncompaction of ventricular myocardium(NVM) in left ventricle with iSLICE technology. Methods Fourteen patients with NVM and 14 healthy subjects were selected. Transthoracic echocardiography was performed with Philips iE33 machine and full volume data of three-dimensional image was collected from which iSLICE image had been obtained. Area of noncompaction inside left ventricle was measured in an echo graphic system made in our laboratory. The ratio of no, compaction area(RNCA) was calculated in every slice of left ventricle form apex slice to the slice at tip of mitral valve level. Results Broad distribution of noncompaetion was observed from iSLICE image. Full of noncompaction tissue was seen in the apex in NVM group(RNCA = 100 % ). Compared with control, RNCA in the NVM group were also higher (62.14%~40.00%) at slices from papillary muscle level to the slice at tip of mitral valve level than that in the control( P 〈0. 001). Conclusions iSLICE image might provide a modality for analyzing structure of noncompaction of left ventricular chamber in patients with NVM quantitatively.
