摘要
目的:探讨超声心动图技术对孤立性心室致密化不全(IVNC)的诊断价值。方法:应用彩色多普勒超声心动图多切面探查肌小梁隐窝内血流以及有无附壁血栓形成,测量左室射血分数(LVEF)。结果:①受累节段的室壁明显分为薄的致密外膜层和厚的致密化不全内膜层;彩色多普勒显示,小梁隐窝血液与左心腔相通;②2例探及小梁隐窝内附壁血栓;10例患者LVEF降低穴0.42±0.07雪;6例合并二尖瓣返流;③致密化不全共累及45个节段,73.3%(33/45)位于心尖段、侧壁、下壁及后壁的中段。结论:IVNC具有典型的超声心动图特征,彩色多普勒超声心动图技术对其具有重要的诊断价值。
Objective: To assess the diagnostic value of color Doppler echocardiography for isolated ventricular non-compaction (IVNC). Methods: Eleven patients with IVNC were studied using color Doppler echocardiography. Left ventricular ejection fraction (LVEF) was calculated, and intertrabecular flow and mural thrombi were examined. Results: The affected segments had a prominently compact thin epicardial layer and a non-compact thick endocardial layer. Forward and reversed direct blood flow was from the ventricular cavity into the intertrabecular recesses. Mural thrombi were found in 2 patients. Ten cases showed reduced LVEF (0.42±0.07), and 6 cases mild mitral regurgitation. Fourty-five segments were involved by ventricular non-compaction and 73.3% of them (33/45) localized in apex and mid-ventricular segments of both inferior-posterior and lateral walls. Conclusion: IVNC has specific echocardiographic characteristics and the technique of color Doppler echocardiography plays a pivotal role in its diagnosis.
