海藻酸钙膜对兔胫骨骨缺损的修复作用 被引量:7

摘要 目的 探讨海藻酸钙膜(calcium alginate film, CAF)在骨创伤或骨缺损应用中的组织学特点。方法 以家兔为研究对象,在其双侧胫骨形成洞形骨缺损,实验侧骨缺损上覆盖CAF,对照侧覆盖胶原膜或不覆盖任何膜,分层缝合,随诊饲养。分别在术后1,2,4,6,8周时取出胫骨,进行大体观察、组织切片观察以及行骨形态发生蛋白(BMP)、骨钙素和血管内皮生长因子(VEGF)免疫组织化学染色后,对此3种骨诱导因子含量进行测定。结果 CAF4~6周吸收,胶原膜6~8周吸收;前者巨噬细胞反应程度轻,后者反应程度重;前者大体观骨缺损愈合表面平整,后者表面不连续或有突起,系由过量肉芽组织过渡而来;术后4周内实验侧BMP、骨钙素、VEGF总体含量明显高于对照侧(P〈0.01)。结论 CAF在骨创伤中应用效果好,能更快地促进骨缺损的愈合,异物反应过程短、程度轻,早期骨诱导因子的富集作用强。 Objective To study the effects of calcium alginate film (CAF) on guided bone regeneration (GBR). Methods Circular bone defects with 5ram in diameter were created in tibia bone of 25 rabbits. The defects were covered with a calcium alginate film as experimental site and with collagen membrane or without membrane served as control sites. Gross, histological and immunohistochemical studies analyzed the healing conditions after 1, 2, 4, 6, and 8 weeks post surgically. Results CAF was absorbed in 4-6 weeks, while collagen membrane in 6-8 weeks. In the control sites bone regenerated more slowly, there were more macrophagy and small protrusions and less osteoinductive factors, and CAF was absorbed more slowly than in the experimental site post surgically. Conclusion The results indicate that CAF has better effect than collagen membrane in GBR.
