脑星形细胞瘤组织Topo-Ⅱα与Ki-67及PCNA表达与临床病理因素关系的研究 被引量:2

摘要 目的:研究DNA拓扑异构酶Ⅱα(Topo-Ⅱα)、Ki-67和增殖细胞核抗原(prolifera-ting cell nuclear antigen,PCNA)在中枢神经系统星形细胞瘤中的表达,并结合多种临床因素探讨其与肿瘤恶性程度及预后的关系。方法:应用SABC免疫组化技术,检测32例星形细胞瘤Topo-Ⅱα、Ki-67和PCNA的表达,并对其与恶性程度的关系及可能影响预后的因素进行统计学分析。结果:星形细胞瘤Topo-Ⅱα、Ki-67和PC-NA的表达阳性率在低度恶性组中为28.5%、21.4%和28.5%;在高度恶性组中为100%、100%和88.8%,两组之间的差异均有统计学意义(Topo-Ⅱα组χ2=23.87,P<0.01,Ki-67组χ2=18.16,P<0.01,PCNA组χ2=13.41,P<0.01);单因素条件Logistic回归分析显示,患者年龄、恶性程度、肿瘤切除范围、KPS、肿瘤Topo-Ⅱα和PCNA对2年生存率有意义;多因素分析表明,肿瘤的恶性程度、术前患者KPS及Topo-Ⅱα的高表达是影响星形细胞瘤患者预后的主要因素。结论:Topo-Ⅱα、Ki-67和PCNA在星形细胞瘤中的表达与其恶性程度相关,肿瘤的恶性程度、术前患者KPS及Topo-Ⅱα的高表达与患者的预后生存相关,可作为判断与预测星形细胞瘤患者预后的重要指标。 OBJECTIVE: To examine the expressions of Topoisomerase Ⅱα and PCNA in astrocytoma, and investi- gate the relevance of Topo-Ⅱα, Ki-67, PCNA and several clinical factors with tumor malignant degree and prognosis. METHODS: The SABC immunohistochemical staining method was used to examine the expressions of Topo-Ⅱα, Ki-67, PCNA in 32 human astrocytomas, and the correlations of the three factors with malignant degree and the factors affecting prognosis were evaluated statistically. RESULTS: The positive expression rates of Topo-Ⅱα, Ki-67, PCNA in low grade group were 28.5%, 21.4% and 28.5%, respectively;100%, 100% and 88.8%, respectively, in high grade astrocytomas, and there was a significant difference between the two groups. The patient's age, tumor resection cope, the malignant degree, the preoperative KPS, the Topo-Ⅱα and PCNA expression were correlated with the survival by univariate analysis. The results of multivariate logistic analysis showed that the malignant degree, the preoperative KPS score, the Topo-Ⅱα high expression were significantly correlated with the prognosis. CONCLUSIONS: The expressions of Topo-Ⅱα, Ki-67, PCNA are correlated with astrocytoma malignant degree and prognosis. The malignant degree, the preoperative KPS score, the Topo-Ⅱαhigh expression were prognostic markers of astrocytoma.
