BMP/DBM修复关节软骨缺损的实验研究 被引量:9

摘要 作者旨在对骨形态生成蛋白/脱钙骨基质(BMP/DBM)复合体在关节软骨缺损修复中的作用进行探讨。在16只雄性兔双侧股骨髁间凹各做一直径4mm,深2.5mm的全层关节软骨缺损。16个右膝缺损区植入BMP/DBM,8个左膝缺损内单独植入DBM,另外8个左膝为空白。术后12周处死动物,取材,石蜡包埋,连续切片,做苏木素一伊红,甲苯胺蓝及safranin-O染色。结果显示,在BMP/DBM组,植入物完全被吸收并被新生软骨替代,14例(14/16)愈合,且12例safranin-O阳染,新生软骨厚度和特征与邻近正常软骨相似。DBM组只有2例(2/8)愈合,safranin-O染色全部阴性。空白组未见新生软骨。结果说明,BMP/DBM能在关节腔内诱导软骨再生,并能以透明软骨修复全层关节软骨缺损。 Theauthorsstudiedtheefectsofbonemorphogeneticprotein/demineralizedbonema-trix(BMP/DBM)fortherepairofdefectsofthearticularcartilage.In16malerabbits,defectsof4mmdiameterand2.5mmdepthwerecreatedthroughfulthicknessofthearticularcartilagebilateralyintheintercondylargrooveofthefemur.BMP/DBMwasinsertedin16oftherightkneedefects.Overtheleftside,DBMdiscsweregraftedalone,andinanother8,thedefectswereleftaloneascontrols.At12weekspostoperatively,theanimalsweresacrificedandspecimensofthedistalfemurwereprocuredandpreparedforhistologicstudywithHEstain,toluidineblueandsafranin-Ostaining.IntheBMP/DBMgraftedgrouptheresultsshowedcompleteabsorptionofBMP/DBMandsubstitutionbynewcartilagetissue.14ofthe16kneedefectswerehealedand12hadpositivestainingwithsafranin-O.Thethick-nessandfeaturesofthenewarticularcartilagewassimilartothatofthenormal.InDBMgraftedgroup,thethicknessofthenewarticularcartilagewasnotashealthyasintheBMP/DBMgraftedgroup.Noneofthespecimenwaspositvelystainedwithsafranin-O.Only2ofthe8defectshealed.Inthecontrols,nonewcartilagetissuewasseen.Conclusively,BMP/DBMmayinducecartilageregenera-tionandrepairthearticularcartilagedefectswithhyalinecartilageoverthejointsurface.
