摘要
目的:临床应用Leeds-Keio人工韧带辅助自体髌腱(中1/3)移植重建前交叉韧带,治疗前交叉韧带断裂,并观察其临床效果。方法:1995年10月至1998年2月间,应用Leeds-Keio人工韧带辅助自体骨-髌腱(中1/3)-骨-髌前筋膜-股四头肌腱瓣重建前交叉韧带32例。术后实施积极的康复训练。结果:术后随访26例、7～9年(平均8年),Lysholm功能评分由术前平均67分提高到术后平均87分,Noyes功能评分由术前平均147分提高到术后平均230分,患者对手术的主观评价满意度平均为86%(70～99%)。运动员患者术后第6个月恢复正式训练、比赛。16例术后(平均20个月)在行内固定物取出时进行了关节镜检查,重建韧带外形及张力良好。术后再伤导致人工韧带断裂1例,1例出现明显异物反应取出人工韧带。结论:应用Leeds-Keio人工韧带辅助自体髌腱(中1/3)重建前交叉韧带的临床治疗效果满意,术后可采用快速康复程序,康复快,运动员患者术后早期即可恢复训练与比赛。提示应用Leeds-Keio人工韧带辅助自体髌腱(中1/3)移植重建前交叉韧带可行,适合复合韧带损伤患者和期望迅速恢复运动训练的运动员。
Objective To observe the clinical outcome of anterior cruciate ligament（ACL） reconstruction with patellar tendon（middle third） autograft augmented with Leeds- Keio artificial ligament. Methods From October 1995 to February 1998, a total of 32 patients suffered from ACL rupture underwent ACL reconstruction using bone - patellar tendon（ middle third） - bone - quadriceps autograft augmented with Leeds - Keio artificial ligament and followed by accelerated rehabilitation program. Results 26 of 32 patients were followed - up for 7 years to 9 years （average 8 years）, patients＇ mean Lysholm score was improved from 67（41 - 85）preoperatively to 87（80 - 99） postoperatively, and Noyes score from 147 to 230. Subjective satisfaction for operation was 86%（70 - 99% ）, only 1 case had foreign body reaction. The mean duration for athletes to fully resume competition was 6 months postoperatively. The reconstructed ACLs were reexamined with arthroscopy in 16 cases 20 months（average） after surgery and the reconstructed ACLs revealed good appearance and tension with an exception of 1 case, in which Leeds - Keio artificial ligament was ruptured due to reinjury. There was foreign body reaction in 1 case, and the artificial ligament had to be removed 9 months after surgery. Conclusion Our clinical findings suggest that the reconstruction of anterior cruciate ligament with autogenous tendon augmented with Leeds - Keio artificial ligament followed by an accelerated rehabilitation program is a satisfactory treatment for patients with ACL rupture and complex ligament injury and for athletes who expect to resume high level physical activity early.
