摘要
目的:研究临床随意皮瓣术后,实验组和对照组皮瓣存活情况,一氧化氮(NO)及超氧化物歧化酶(SOD)、丙二醛(MDA)量的变化,探讨和明确L-精氨酸(L-arg)对随意皮瓣的存活有无影响,有何影响?其作用机制是什么等问题。方法:采用随机双盲实验方案,实验组每日给予L—arg15克,对照组每日给予甘氨酸25克,实验共持续7天,在术后24小时拔引流管时取皮瓣组织检测NO、SOD、MDA的含量,术后7天观测两组皮瓣存活长度。结果:实验组术后24小时NO含量高于对照组(P<0.01),实验组SOD含量高于对照组(P<0.01),而实验组MDA含量低于对照组(P<0.01),实验组皮瓣存活比例也明显高于对照组(P<0.01)。结论:L-arg对随意皮瓣存活有促进作用。其作用机制可能是增加外源性L-arg的量,促进了机体组织中NO的生成,进而使组织中SOD增多,并降低了机体组织中的MDA,改善皮瓣血供,减轻组织缺血,促进了创面愈合,增加了皮瓣存活。以上结果与动物实验一致。
Objective: This paper aims to investigate the clinical influence of L- arginine on Random pattern skin flap, through the changes of the NO, SOD, MDA, and the survival length of flap in controlled group and experimental group. Methods: Two- blind text, After operation, the parents were giving L- arg 15g in experimental group, while the parents were giving 25g G- arg in controlled group Imidiately. the NO, SOD, MDA of the flap were examined after twenty- four hours of the operation, and the survival length of flap in two groups were examined after seven days of the operation. Results: The NO and SOD of the flap after twenty - four hours of the operation in experimental group were significantly higher than the controlled group ( P 〈 0. 01 ), the MDA in experimental group was significantly lower at twenty - four hours after the operation compared with the controlled group, the survival length of the flap in the L- arginine - treated group significantly increased ( P 〈 0.01) compared with controlled group. Conclusion. Supplement of exogenous arginine may be beneficial to the random skin flap in promoting the survival length. The higher survival length of the flap might owes to the mechanism: the supplement of exogenous arginine promote the tissue to produce more NO, and than the SOD in the flap produces more too, while the MDA in the flap produces less compared with the controlled group. All the changes above improve the microcireulation, promote wound healing and improve the survival length of the flap in experimental group. The result is same with result in SD mouse.
