NIDDM病人血小板聚集、第Ⅷ因子相关抗原的临床研究 被引量:6

摘要 本文对23例初发Ⅱ型糖尿病(NIDDM)病人进行口服降糖药吡磺环已脲(glipizide)治疗前后的血小板聚集、第Ⅷ因子相关抗原(ⅧR:Ag)和纤维蛋白原进行动态研究。结果表明:NIDDM病人在临床未发现微血管病变时已有血小板聚集率升高,血小板聚集率与空腹血糖、糖化血红蛋白A_1(GHbA_1)无关,与ⅧR:Ag呈明显正相关。glipizide治疗后血小板聚集率明显下降,提示血小板功能异常可能参与NIDDM微血管病变的发生;内皮损害会加剧血小板聚集功能,glipizide对抑制血小板聚集功能有一定作用。 Platelet aggregation, plasma factor Ⅷ-related antigen (Ⅷ R:Ag) and fibrinogen were measured in 23 newly diagnosed diabetic patients after two weeks on diet alone, and at the 2nd, 4th and 6th month during treatment with glipizide. 9 of the 23 patients had microangiopathy and the other 14 had not. On diet alone, the maximal platelet aggregation in diabetic subjects was increased significantly in response to various concentrations of adenosine diphos-phate or adrenaline as compared with those in normal controls (P<0.05), but there was no significant difference between the diabetics with and without microangiopathy. Although the platelet aggregation exhibited a significant positive linear correlation with the levels of Ⅷ R:Ag in diabetic patients, it did not correlate to fasting blood glucose and glycosylated hemoglobin A1 (GHbA1). After two months' treatment with glipizide, the platelet aggregation and the level of fibrinogen of the patients were significantly reduced with an accompanying fall of fasting blood glucose and GHbA1 when compared with the pretreatment values (P<0.05). And the diabetics were under good control during glipizide therapy. These results suggested that the abnormal platelet hyperaggregation was the result of endothelial damage and was. further enhanced by high level of Ⅷ R:Ag; it may play a role in the development of diabetic microangiopathy. The beneficial effect of glipizide on platelet abnormalities seems to be due to its direct action rather than its, hypoglycemic effect.
