FPA与D-dimer二种分子标志物检测对糖尿病肾病早期诊断的意义研究 被引量:2

摘要 目的研究糖尿病肾病患者FPA与D-dimer的变化,探讨两种分子标志物的检测对早期诊断糖尿病肾病的意义。方法选择糖尿病肾病患者30例作为研究对象,分别测定血浆PT,APTT,Fg;尿液FPA;血清D-dimer水平,将所得结果进行逐一比较。结果DN组与DM组、DN组与健康对照组比较,PT,APTT缩短。Fg增加,但经统计学处理,各组间无明显差异(P<0.05)。FPA与D-dimer水平测定中,DN组分别为55.839±16.623 ng/mgCr及1.794±0.84 mg/L,经统计学处理,DN组与其它两组比较均有明显增高(P<0.01)。结论(1)凝血功能筛选实验(PT,APTT,Fg)测定值变化不能反映凝血系统活化状态,对糖尿病肾患者的早期诊断缺乏敏感性。(2)FPA与D-dimer检测变化非常明显,是凝血因子活化指标,能敏感的直接反映凝血与纤溶阶段的活化,是诊断早期糖尿病肾病的较敏感指标。 Objective To state change of coagulation mechanism in diabetic nephropathy(DN)and to probe the examination meaning for the disease develop and thrombosis. Methods Thirty patients with diabetic nephropathy as the objects of research. Thirty patients with diabetes and thirty healthy volunteer as control group.PT.APTT and Fg were detected by eogulation instrument; FPA was edtected by HPLC;D-D of serum was detected by ELISA, and the results were compared to tbe groups.Results Comparing DN with DM and DN with common control groups.TP.APTT were shorten,Fg was increase.Tbere were no significant differences between the groups(P 〉 0.05) .The measure of FPA and D-D in DN,FPA was 55.839±16.623 ng/mg Cr and D-D was 1.794±0.848 mg/L,comparing DN with other two groups, they were significantly different( P 〈 0. 01 ). Conclusion ( 1 )The scalp test parameters changes of coagulation function only reflect coagulable protein level totally in viro,but not the activation states of the coagulation system, so it is not direct correlation with tendency of thrombus formation. (2)The changes of FPA and D-D were very obviously in DN,and were activate index of coagulable factor, and can sensitive directly reflect the activation of coagulation and Fibrinolysis in coagulation process.
