摘要
本文测定１８例放置甲地孕酮硅橡胶阴道环发生突破性出血妇女外周血及子宫内膜组织中６—酮—前列腺素Ｆ１α（６－ｋｅｔｏ－ＰＧＦ１α）与血栓素（ＴＸＢ２）含量，以２１例放甲硅环未发生出血妇女及２１例未放环妇女为对照。结果表明：各组外周血Ｅ２和Ｐ水平无显著差异。使用甲硅环发生突破性出血（ＢＴＢ）者外周血中６－ｋｅｔｏ－ＰＧＦ１α、ＴＸＢ２合量及６—ｋｅｔｏ－ＰＧＦ１α／ＴｘＢ２比值在增殖期明显高于放环未出血组及正常组。发生ＢＴＢ者子宫内膜组织中６－ｋｅｔｏ－ＰＧＦ１α明显高于未出血组。提示：前列腺素在甲硅环所致ＢＴＢ中有一定作用。本文结果还表明，应用炔雌醇和萘普生治疗ＢＴＢ，其有效率分别为９１．１％和７６．７％，提示除６—ｋｅｔｏ－ＰＧＦ１α和ＴＸＢ２参与引起ＢＴＢ外，雌二醇可能也是ＢＴＢ发生的部分机制之一。
Prostaglandins (6-keto-PGF1α and TXB2)levels in peripheral blood during breakthrough bleeding(BTB) were determined in 18 women bearing megestrol acetate-releasing silastic intravaginal ring(SVR-MA). The concentrations of 6-keto-PGF1α and TXB2 in endometrial tissue were also measured in 21 SVR-MA bearing women with no BTB (nonbleeder group) and in 21women not bearing intravaginal ring (normal group) served as control group. There was no difference in plasma levels of E2 and P among all groups. The peripheral blood values of 6-keto PGF1α TXB2 and the ratio of 6-keto-PGF1αto TXB2 of breakthrough bleeders in proliferative phase were signifcantly higher than those of both normal and nonbleeder group. The content of 6-keto-PGF1α in the endometrial tissue of breakthrough bleeders was significantly higher than that of SVR-MA bearing women with no BTB. The results suggest that prostaglandins may play a role in the pathogenesis of breakthrough bleeding.Therapeutic effects of ethinyloestradiol (EE) and naproxen in BTB womem were evaluated.Both are satisfactorily effective. The former (effective rate 91. 1 %) is better than the latter(76.7%). It indicates that besides prostaglandins, estradiol also plays a role in the pathogenesis of BTB.
