促甲状腺素释放激素治疗高原失血性休克并发肺水肿

摘要 目的 在海拔 376 0m的西藏拉萨研究促甲状腺素释放激素 (TRH)对高原创伤性休克合并肺水肿的急进高原大鼠的治疗作用。方法 初进高原大鼠 35只 ,均分为 5组 :假手术对照组 ,失血性休克组 ,失血休克 +肺水肿组 ,失血性休克 +肺水肿+TRH治疗组 ,失血性休克 +肺水肿 +等容量平衡盐液组。大鼠从股动脉放血至血压 (MAP) 5 0mmHg ,5 μL/10 0 g油酸iv ,维持 1h ,制造失血性休克合并肺水肿模型。观察治疗后 15 ,30 ,6 0 ,12 0min的血流动力学指标变化 ,30和 12 0min的血气指标变化以及 12 0min肺脑含水量变化。结果  5mg/kgTRH有良好的抗休克作用 ,给药后MAP升高 ,血流动力学指标改善 ,酸碱平衡紊乱减轻。肺湿 /干重比减少。 OBJECTIVE To study the effects of thyrotropin-releasing hormone(TRH) on hemorrhagic shock with pulmonary edema in the rat just entering into high altitude. METHOD 35 SD rats, transported to LaSa, Tibet, 3760 meters above the sea level, were anesthetized with sodium pentobarbital (30mg/kg, ip). Hemorrhagic shock model with pulmonary edema was induced by hemorrhage(50 mmHg) plus intravenous injection of oleic acid(5μL/100g wt) for 1 hour. Rats were equally divided into 5 groups(n=7 each group): sham-operated group, hemorrhagic shock alone, hemorrhagic shock with pulmonary edema(HSPE), HSPE plus TRH(5 mg/kg) and HSPE plus lactate ringer solution( 4mL/kg). Hemodynamic parameters including mean arterial blood pressure(MAP), left intraventricular systolic pressure (LVSP) and the maximal change rate of intraventricular pressure rise or decline (±dp/dt max) were observed at 15, 30, 60 and 120 min, blood gases were observed at 30 and 120 min, and the water content of lung and brain was determined at 120 min after drug administration. RESULTS TRH(5 mg/kg)significantly increased MAP, LVSP and ±dp/dt max, improved the variables of blood gases and decreased the water content of lung and brain. CONCLUSION TRH has a good effect in the treatment of hemorrhagic shock with pulmonary edema at high altitude.
