实时三维超声心动图对经导管闭合房间隔缺损术后的观察

摘要 目的 :探讨实时三维超声心动图 (RT- 3DE)对经导管闭合房间隔缺损 (ASD)术后观察的价值。方法 :对 15例ASD患者术后封堵器的位置、形态、塑型、大小及运动进行 RT- 3DE观察 ,并与手术结果进行比较。结果 :RT- 3DE显示封堵器呈双盘状 ,测得封堵器伞腰直径为 2 7± 5 mm ,与实际封堵器大小 (2 6± 5 mm )比较无显著差异 (P>0 .0 5 ) ;封堵器相对房间隔无移位 ,随心脏运动 12例无扩展和收缩 ,形状也无变化 ,3例有变化。结论 :RT- 3DE成像速度快 ,模拟外科手术路径实时显示封堵器的位置、形态和塑形以及和周围组织的关系 ,可作为 ASD介入治疗术后疗效评价的重要手段。 AIM: To evaluate the application of real-time three-dimensional echocardiography (RT-3DE) to the observation of curative effect of interventional treatment of Atrial septal defect (ASD) with Amplatzer closure devices. METHODS: Sizes, shapes, sites and movements of devices after the treatment were observed with 3DE in 15 secudum ASD patients. RESULTS: RT-3DE could clearly visualize the device's double-disc like shapes and sites in 3 dimensions. There was no significant difference between the measurements of occluder waist sizes with 3DE and actual ones (27±5mm vs 26±5 mm, P>0.05). 12 devices did not move in cardiac cycles and their shapes did not change except those of 3 cases. CONCLUSION : RT-3DE is a high frame rate imaging technology, which can provide clear real-time illustration of shapes, sites, remodeling and motion of Amplatzer closure devices by simulating an operation. It can be used as an important method for evaluating interventional treatment of ASD.
