摘要 目的 探讨HLA抗原在肝癌中的表达水平及与病程、预后等的关系。方法 应用 5种HLA及相关分子单抗 ,采用免疫组织化学方法 (ABC法 )对 93例肝细胞肝癌及癌旁组织的石蜡切片进行检测。结果 HLAI类抗原在癌旁正常肝细胞中不表达或低表达 ,在癌细胞中表达明显增强 ;肝癌中HLAI类抗原表达增强的患者生存率较其他患者高 ,肝癌中I类抗原表达阳性的患者较表达阴性的患者生存率高。未发现肝癌细胞HLAI类抗原表达的增强与年龄、AFP、HBsAg、肝硬化、瘤体大小及病理分级有关。HLAII类抗原在肝癌及癌旁细胞中均表达较弱或不表达。结论 HLAI类抗原在肝癌中表达较癌旁细胞增强 ,并有利于患者的预后。肝癌中HLAI类抗原表达增强这种有别于其他肿瘤的分子机制及意义等问题还需进一步研究。 Objective To investigate the expression of HLA in human hepatocellular carcinoma (HCC) and its clinical significance. Methods Ninety three cases of formalin fixed paraffin embeded tumor tissues and their corresponding autologous normal specimens were tested, by 5 monoclonal antibodies (mAbs) using ABC immunohistochemical method. Results Normal hepatocytes show weak or no expression of HLA class I. Meanwhile HCC has upregulated expression apparently than adjacent normal hepatocytes. Upregulated expression of HLA class I in liver carcinoma lesions is significantly ( P =0.02) associated with an improved prognosis, and the survival rate of the patients that show positive HLA class I antigen expression in HCC is higher than that of negative expression. No statistically significant difference is found between the upregulation in HCC with their corresponding adjacent normal liver according to age, AFP, HBsAg, liver cirrhosis or course of illness. The expression of HLA class II molecule is slight higher in HCC (but no statistical significance). Conclusion These results are compatible with the possibility that induction of HLA class I antigen expression in liver carcinoma cells may render them susceptible to CTL mediated recognition and lysis. The molecular mechanism under the upregulated expression of HLA class I in HCC that is different from other tumors and its significance needs to further research.
