摘要
目的 观察妊高征 (PIH)患者红细胞膜Ca2 + -ATP酶 (PMCA)活性改变及其与胰岛素的关系。方法 以 2 8例血压正常孕妇及 2 8例中重度妊高征患者为研究对象 ,采用生化方法提取红细胞膜 ,测定红细胞PMCA活性 ;采用放射免疫分析法测定血清胰岛素水平 ,观察两组PMCA活性差异及PMCA与胰岛素的关系。结果 妊高征组红细胞PMCA活性明显低于正常孕妇组 (PIH 1 1 1 1± 0 30 6 μmol·pi/mg·h ;NP 1 6 72± 0 4 34μmol·pi/mg·hP <0 0 5 ) ,且与平均动脉压成负相关 (r=- 0 81 8,P <0 0 5 ) ;妊高征组血清胰岛素水平明显高于正常孕妇组 (PIH 2 0 4± 0 37lnμU/ml;NP 1 85± 0 34lnμU/ml,P <0 0 5 ) ,且与红细胞PMCA活性成负相关 (r=-0 781 ,P <0 0 5 )。结论 PIH患者红细胞PMCA活性降低引起细胞内游离Ca2 + 浓度升高 ,导致妊高征发生 ;PIH患者血清胰岛素水平明显升高 ,红细胞PMCA活性明显减低 ,二者具有显著相关性 。
Objectives To study the activity of calcium adenosine triphosphatase of red blood cell membrane(PMCA)of patients with pregnancy-induced hypertension(PIH)and the relationship betwenn PMCA and sera insulin.Methods Twenty-eight patients with moderate or severe PIH and 28 pregenant women of normal blood pressure(NP) were studied.Hemoglobin-free red bloold cell ghosts were prepared from the heparinized blood and the activity of calcium adenosine triphosphatase was determined chemically.Concentration of sera insulin was measured by radioimmunological assay.Results The average PMCA activity of PIH was lower than NP significantly(1 111±0 306 μmol·pi/mg·h vs.1 672±0 434 μmol·pi/mg·h P <0 05)and the PMCA activity was negatively correlated with MPA(r=-0 818, P <0 05).The sera insulin of PIH was more than NP significantly(2 04±0 37 ln μU/ml vs, 1 85±0 34 ln μU/ml, P <0 05),which showed negative correlation with erythrocyte PMCA activity(r=-0 781, P <0 05).Conclusions Inhibition of PMCA activity of erythrocyte in pregnancy-induced hypertension might increase cytoplasmic free calcium and contribute to the development of pregnancy-induced hypertension.The correlationship between elevation of sera insulin and decreasing PMCA activity indicates the presence of insulin resistance in pregnancy-induced hypertension.
