系统性红斑狼疮患者外周血γ-干扰素和白介素-10阳性细胞的研究 被引量:8

摘要 目的:研究活动性系统性红斑狼疮(SLE)患者外周血中,γ-干扰素(IFN-γ)、白介素(IL)-10阳性细胞在SLE病理机制中的意义。方法:采用微量全血标记法结合胞内细胞因子标记技术,以CD4、CD8分子为T细胞表面标记,经三色流式细胞仪检测未经刺激和经刺激培养后的SLE患者外周血T细胞亚群比率。结果:SLE患者外周血的CD4+及CD8+T细胞中IFN-γ-IL-10+细胞比例均显著升高,IFN-γ+IL-10-细胞与IFN-γ-IL-10+细胞的比值明显降低,且IFN-γ+IL-10+双阳性细胞显著增多;SLE外周血的CD4+及CD8+T细胞中IFN-γ+IL-10-细胞亚群对离子霉素刺激的反应性明显降低;肾上腺皮质类固醇治疗可以降低SLE外周血CD4+及CD8+T细胞中IFN-γ-IL-10+细胞亚群及CD4+T细胞中IFN-γ+IL-10+细胞亚群的比例,提高IFN-γ+IL-10-/IFN-γ-IL-10+细胞亚群比值。结论:SLE患者体内存在明显的T细胞亚群平衡失调,表现为分泌IL-10的T细胞亚群比例增高和Tr1细胞增多、Th1/Tc1细胞对刺激的反应存在缺陷。皮质类固醇对T细胞亚群数量及比例有一定的影响。 Objective:To investigate IFN-γ + and IL-10 + cells of peripheral blood cells in patients with systemic lupus erythematosus(SLE)and explore the role of Th1/Th2and Tc1/Tc2in the pathogenesis of SLE.Methods:The trace whole blood technology was applied to detect intracellular cytokines.The cultured peripheral blood cells were treated with two groups of stimulators(PHA+PMA,Ionomycin+PMA).The levels of CD4 + IFN-γ + IL-10 - /CD4 + IFN-γ - IL-10 + cells and CD8 + IFN-γ + IL-10 - /CD8 + IFN-γ - IL-10 + cells were observed by three-color flow cytometry.Results:The number of CD4 + IFN-γ - IL-10 + and CD8 + IFN-γ - IL-10 + subsets in SLE were higher than those in normal controls,so as the IFN-γ + IL-10 + subset.The ratio of IFN-γ + IL-10 - /IFN-γ - IL-10 + was significantly lower than controls.The reaction of IFN-γ + IL-10 - subsets to ionomycin was significantly lower than controls.The therapy of glucocorticoid could reduce the numbers of IFN-γ - IL-10 + and IFN-γ + IL-10 + subsets.The ratio of IFN-γ + IL-10 - /IFN-γ - IL-10 + was increased by the same therapy.Conclusion:The imbalance of T cell subsets in peripheral blood cells exists in patients with SLE,with the manifestation of high number of IL-10 + cells and T regulatory cells.The reaction of Th1/Tc1subsets to stimuli is significantly lower than controls.The therapy of glucocorticoid can partly change the subset number and ratio.
