应用畸变产物耳声发射检测早期发现接触噪声人群的听力损失 被引量:3

摘要 目的:从听力学角度评价畸变产物耳声发射检测对噪声性听损伤早期发现的价值。方法:利用声级计测定某食品包装材料公司生产车间噪声水平,同时利用纯音测听、声导抗、耳声发射、对生产车间接触噪声的43名工名进行听力检测并同时进行外耳检查和噪声接触史调查。结果:生产车间9个机器点的噪声水平平均93.7dB(A),超过国家规定标准85dB(A),受检的43名工名接触噪声史1~12个月,平均7个月,平时不适有15名。声导抗检测全部正常,耳声发射检测不合格的有23名,占53%,包含纯音测听不正常的有7名11耳,占16%,其中V形曲线8耳,坡形曲线3耳。16名纯音测听正常而耳声发射不合格,其中单耳不合格7名,双耳不合格9名。23名听力检测异常,其中7名纯音测听呈噪声性聋早期、中期听力损伤改变,16名纯音测听正常而耳声发射不合格。结论:利用畸变产物耳声发射技术检测接触噪声的人群,对早期发现噪声性听力损失具有一定价值。 AIM:To assess the value of distortion product otoacoustic emission (DPOAE) in screening the early noise induced hearing loss (NHIL) according to audiology. METHODS:Forty three workers in a food package manufactory received the determination of audiology and the measure of external ear and the survey of contacting noise with pure tone, acoustic immitance and DPOAE. Using acoustic metrology the noisy level in the workshop. RESULTS: The average noise level of nine machines was 93.7 dB(A), exceeding the national standard . The time of contracting noise was 1-12 months and the average time is 7 months. Fifteen workers felt unwell in normal time.Forty three workers were normal with acoustic immitance and 23 workers were abnormal with DPOAE screening, in which 11 ears of 7 workers were abnormal with pure tone measurement, including 8 V sketch and 3 descent sketch. Sixteen workers were normal with pure tone measurement but abnormal with DPOAE, in which 9 single ears of 7 workers and double ears of 9 workers were abnormal Twenty three workers were abnormal in hearing, 7 of which were early and middle stage of noise induced hearing loss change with pure tone audiometry. Sixteen workers were normal with pure tone audiometry but abnormal with DPOAE. CONCLUSION:DPOAE is a valuable method to detect people contracting noise and discover NHIL early.
