摘要
目的:分析颈椎病患者退变颈椎基质骨代谢变化,并探讨其退变的生物学机制。方法:收集16例手术治疗的脊髓型颈椎病患者的退变椎体,并随机取其中6例的外周血;另取5例急性脑死亡者正常椎体作为对照。放射免疫法测定透明质酸(HA)、层粘连蛋白(LN)、Ⅲ型前胶原、Ⅳ型胶原含量;全自动生化分析仪测定钙、磷和总蛋白质含量。结果:颈椎病患者的退变椎体中HA、LN、Ⅲ型前胶原、Ⅳ型胶原均较正常对照骨显著降低(P<0.01),并且排除了血液因素引起退变椎体骨基质成分的变化;退变椎体骨组织中钙、磷含量较正常骨中的含量也显著降低(P<0.01)。结论:颈椎在退变过程中骨基质成分明显减少,从而引起颈椎骨韧性、硬度的减低,可能是导致颈椎生物力学的改变和退变逐渐加重的生物学机制之一。
O bjective:To investigate the biological mechanisms of the degeneration of cervica l spine in cer-vical spondylotic myelopathy(CSM)by analyzing the alterations of bone matrix metabolism.Method:Sixteen de-generative cervical vertebra and6bl ood samples from CSM cases were collected.The contents of Hyaluronic acid(HA), Laminin(LN),Pro-collagenⅢ,CollagenⅣwere detected by radioimmunoassay.Calciu m,phosphate and protein levels were detected by automatic biochemical analyzer,f ive normal vertebra by acute brain deoth cases as control groop,five normal ver tebra by acufe brain death cases as control groop.Result:Ruling out the inferenc e of blood,the contents of HA,LN,Pro-collagenⅢand CollagenⅣwere very significa ntly lower in degenerated CSM vertebra than the control,so as the levels of calc ium and phosphate.Conclusion:Bone ma-trix components are obviously decreased du ring the degeneration of cervical vertebrae,resulting in the allevia-tion of bo ne tenacity and hardness.The above finding should be one of the biological mecha nisms of degener-ation of cervical vertebra and deterioration of cervical spine biomechanics in CSM.
