The effect of krypton laser photocoagulation before laser in situ keratomileusis on the postoperative retinal stability

摘要 目的 :观察LASIK术前预防性氪离子激光光凝对术后视网膜稳定性的影响。方法 :LASIK术前 ,间接检眼镜检查眼底 ,对视网膜裂孔或病变大于 2PD的 658例 (688只眼 )患者行氪离子激光光凝 ,观察其视网膜稳定情况 ;对视网膜病变小于 2PD的 71 2例 (756只眼 )患者密切观察其视网膜病变的变化 ,必要时于LASIK术后补行氪离子激光。结果 :LASIK术后 6个月随诊 ,术前行氪离子激光光凝的 2 38只眼 ,术后视网膜情况稳定 ;术前视网膜病变范围小于 2PD有 2 70只眼 ,有 5只眼于LASIK术后补行氪离子激光光凝。结论 :LASIK术前 ,对视网膜裂孔或病变 >2PD的患者应行激光光凝治疗 ,而对视网膜变性 <2PD的患者可暂不行激光光凝治疗 ,密切随诊其视网膜病变情况。 Objective:To investigate the effect of krypton laser photocoagulation before laser in situ keratomileusis(LASIK)in preventing the failure of operation.Methods:LASIK were performed in 13000 patients(25815 eyes)with retinal detachment during 1998 to Aug.2001,and all patients were examined preoperatively with indirect ophthalmoscope,and there were 658 patients(688 eyes)had retinal holes or degeneration>2PD.All these eyes were treated with krypton laser photocoagulation first.2 weeks later after these holes or degeneration closed well,LASIK was performed.The 712 patients(756 eyes)with retinal degeneration less than 2PD were followed up to observe the change of the retina after LASIK,and performed krypton laser photocoagulation if necessary.These patients served as controls.220 patients(238 eyes)in the first group and 264 patients(270 eyes)in the control group were followed up more than 6 months.Results:The retinae were stable in all the 238 eyes of the first group,while in the control group of 270 eyes,5 eyes were performed krypton laser photocoagulation after LASIK.Conclusion:In eyes with retinal hole and degeneration more than 2PD should be treated with krypton laser photocoagulation before LASIK,whereas retinal degeneration less than 2PD may be followed up to observe the change of the retina after LASIK.
