摘要
目的 观察静脉应用丙氨酰谷氨酰胺 (力肽 )对伴肠功能不全危重病患者淋巴细胞及其亚群的影响。方法 4 6例伴肠功能不全危重病患者随机分为力肽组和常规治疗组 ,分别给予等氮等热量营养支持 6天 ,力肽组静脉应用力肽 0 .4 g/ (kg· d)提供谷氨酰胺 ,观察治疗前后患者淋巴细胞记数及其亚群包括 CD+ 3、CD+ 4、CD+ 8、CD+ 4/ CD+ 8、CD+ 3CD+ 2 5、CD- 3CD+ 1 6 + 56 细胞的变化情况 ,取 2 0例健康献血员作健康对照。结果 治疗前危重病患者的淋巴细胞数及其功能均低于健康对照 ,力肽治疗后明显改善 (P<0 .0 1) ,表现为 T淋巴细胞数 (CD+ 3)增加(P<0 .0 5 ) ,其亚群 CD+ 4、CD+ 4/ CD+ 8、CD+ 3CD+ 2 5、CD- 3CD+ 1 6 + 56 (P<0 .0 1)细胞所占比例明显增高。
Objective To investigate the effect of intravenous infused alanyl-glutamine(Dipeptiven)on the lymphocyte immune function of critical ill patients.Methods 46 critical ill patients with gastrointestinal disfunction were randomized to receive either standard nutrition or supplemented intravenous alanyl-glutamine 0.4g/kg,all were treated 6 days.At the 0 and 7 day measured the percent of lymphocyte and its subgroups respectively,included CD 3 +?CD 4 +?CD 8 +?CD 4 +/CD 8 +?CD 3 +CD 25 +?CD 3 -CD 16+56 + cells,20 hedlthy volunteers as the control group.Results Compared with the control group the number of lymphocyte and its function of critical ill patients were all descended.Through treatment the percent of lymphocyte and its subgroups of CD 3 + (P<0.05),CD 4 +,CD 4 +/CD 8 +,CD 3 +CD 25 +,CD 3 -CD 16+56 +(P<0.01)were all increased,and the alanyl-glutamine group was more significantly(P<0.01).Conclusion Intravenous used alany-glutamine could help to improve the critical ill patient's lymphocyte immune function.
