腹腔镜多功能手术解剖器在实验性肝切除术中的应用 被引量:6

摘要 目的 通过动物实验 ,探讨验证腹腔镜多功能手术解剖器用于腹腔镜肝切除术的可行性及可靠性。方法 采用自制的腹腔镜器械 腹腔镜多功能手术解剖器进行 4例家猪的腹腔镜手术 ,在胆囊切除完成后做腹腔镜肝脏部分楔形切除术。记录肝切除时的肝脏离断时间、断肝时的出血量、切除标本大小重量等。在手术前后采静脉血作肝功能检查。术后第 4周处死实验动物 ,大体观察腹腔内情况 ,取肝断面作组织学检查。结果  4例肝叶楔形切除术均顺利完成。实验动物术后第 1天即能自主活动 ,进食好。肝切除时断肝平均时间为 4min。切除肝脏标本体积最大为 5cm× 4cm× 2cm。平均出血量约 3 0ml。肝切除手术后均有不同程度的肝功能改变。术后肝断面病理检查均表现为不同程度的凝固性坏死。结论 腹腔镜多功能手术解剖器是一种有效的肝切除器械 ,操作时无须频繁更换其他手术器械 ,可始终保持术野清洁。对切面小的管道结构处理可靠。在肝胆微创外科有很好应用前景。 Objective: To investigate the feasibility and possibility of the multiple laparoscopic operative device in laparoscopic partial liver resection. Methods: A new laparoscopic operative device was specially designed and applied for laparoscopic liver resection in four porcines. The time of liver isolation, the volume of bleeding, the weight of the resected specimen and operative complications were measured. Venous blood specimens were drawn for the test of liver function before and after operation. Both gross and microscope examination were performed 4 weeks after the operation. Results: All operations were performed successfully. All experimental animal can stand and walk without difficulties the day after operation. The meantime of liver dissection was 4min. The biggest specimen was about 5 cm×4 cm×2 cm. Liver functions were waved after liver resection and the liver section was characterized as coagulative necrosis. Conclusions: Multiple laparoscopic operative device is very effective in laparoscopic liver dissection with the advantages of keeping the operative field clean and needing no frequent instruments alternation. It can also provide the stable management of small liver vessels and ducts, and have a promising prospect in the minimally invasive hepatobiliary surgery.
