摘要
目的 探讨螺旋水刀在腹腔镜肝切除术中应用的可行性。 方法 上腹部放置 4~ 6个trocar,腹腔镜下应用德国ERBE公司生产的螺旋水刀进行断肝 ,肝内管道采用钛夹夹闭 ,切除病灶后肝断面用医用生物蛋白胶处理。 结果 腹腔镜肝左外叶切除术 1例 ,肝局部病灶切除术 4例。手术时间 85~ 2 6 7min ,平均 135min。术中出血量 2 0~ 30 0ml,平均 12 7ml,术中均未输血。术后无胆漏、出血等严重并发症。术后住院 3~ 10d ,平均 5 3d。 结论 腹腔镜下应用螺旋水刀断肝的创面清晰 ,较好地解决了肝切除术中管道处理不够清晰的问题 ,应用于腹腔镜肝切除术可行 ,值得推广。
Objective To investigate the feasibility of water jet technique in laparoscopic hepatectomy (LH). Methods Through 4~6 trocars on the upper abdomen, a Helix Hydro-Jet device (ERBE, Germany) was used for the transection of the liver under laparoscope. The intrahepatic bile ducts were closed with titanium clips and the cut surface of the liver was treated with biological glue. Results There was 1 case of left lateral lobe resection and 4 cases of local liver resection. The operation time ranged 85~267 min (mean, 135 min). The intraoperative blood loss was 20~300 ml (mean, 127 ml). No blood transfusion was required in the surgery. Postoperatively, no bile leakage or hemorrhage occurred. The hospital stay after the surgery was 3~10 days (mean, 5.3 days). Conclusions Laparoscopic liver resection by using the Helix Hydro-Jet device provides excellent visualization of vessels and bile ducts, being a feasible technique and worthy of recommendation.
