针刺对腰神经根压迫症模型大鼠受损腰神经根超微结构及炎性介质的影响 被引量:21

The Influence of Acupuncture over the Ultrastructure of Injured Lumbar Nerve Roots and Inflammatory Mediators of Rat Models of Lumbar Nerve Root Compression

摘要 目的 探讨针刺对受损腰神经根超微结构及炎性介质的影响。方法  SD大鼠随机分为模型组 45只和电针组 45只 ,分别于造模 (电针 )后 15 d和 3 0 d,取压迫区受损腰神经根组织经处理后于电镜光镜下观察 ,同时测定其 PGE2及 5 -HT的含量。结果 电针 15 d组较模型 15 d组神经纤维周围充血、水肿及神经根纤维脱髓鞘变较轻 ,雪旺氏细胞结构基本正常 ;模型3 0 d组神经纤维周围大量炎细胞聚集 ,神经根纤维严重脱髓鞘变 ,神经纤维间隙明显增宽 ,并见红细胞渗出。电针 3 0 d组脱髓鞘变较轻 ,并缓解神经内水肿。模型 15 d和 3 0 d组 ,大鼠受压腰神经根局部组织内 PGE2 含量明显增高 ,而电针 15 d和 3 0 d组 ,大鼠受压神经根局部组织内 PGE2 均较造模组明显下降。模型 15 d、3 0 d组 ,大鼠受压神经根局部组织内 5 -HT含量明显升高 ,而电针 15 d和 3 0 d组 ,大鼠受压神经根局部组织内 5 -HT含量逐渐下降。结论 电针可缓解受损腰神经根周围炎性反应 ,改善腰神经根超微结构 ,并且具有降低压迫腰神经根组织内 PGE2 及 5 -HT含量 ,部分抑制炎症反应的作用。 Objective To explore the influence of acupuncture over the ultrastructure of injured lumbar nerve roots and inflammatory mediators. Methods SD rats were randomly divided into model group and electro acupuncture group, 45 rats in each group. The processed tissues of the injured lumbar nerve roots in the compressed area were observed under the electro microscope and optical microscope 15 days and 30 days respectively after the models (or electro acupuncture) were created (applied); and the contents of PGE2 and 5 HT of the tissues were also determined. Results Compared with the model group, the congestion, edema around nerve fibres and demyelination of nerve fibres in the 15 day eletro acupuncture group were less severe, and the structure of Schwann Cells was basically normal. In the 30 day model group, numerous inflammatory cells accumulated around the nerve fibres, and there're severe demyelination of nerve root fibres, obviously increased distance of nerve fibres as well as exudated erythrocytes. In the 30 day electro acupuncture group, the demyelination of nerve fibres was less severe, and the edema in the nerves was relieved. In the 15 day and 30 day model groups, the contents of PGE 2 in the local tissues around the compressed lumbar nerve roots of rats increased obviously; while in both the 15 day and the 30 day electro acupuncture groups, the contents of PGE 2 in the local tissues around the compressed nerve roots of rats were significantly lower compared with the model groups. In the 15 day and 30 day model groups, the contents of 5 HT in the local tissues around the compressed nerve roots of rats increased obviously; while in the 15 day and the 30 day electro acupuncture groups, the contents of 5 HT in the local tissues around the compressed nerve roots of rats gradually decreased. Conclusion Electro acupuncture could relieve the inflammatory reaction around the injured lumbar nerve root and improve its ultrastructure; it could also partly inhibit the inflammatory reaction by lowering the contents of PGE 2 and 5 HT in the compressed lumbar nerve root tissues.
