摘要 目的 :通过对肝硬化病人的某些营养参数进行评估 ,为临床医师及早发现和治疗营养不良提供证据。 方法 :对 4 3例肝硬化病人进行主观综合性营养评估 (SGA)、直接人体测量和生化参数测定。 结果 :依据SGA ,肝硬化病人分为营养良好、轻~中度及重度营养不良 ,3组间体重指数 (BMI)、三头肌皮皱厚度 (TSF)、上臂肌围 (AMC)、肌酐身高指数 (CHI)及转铁蛋白 (TRF)均有显著差异。其中TRF和CHI对重度营养不良的识别率高达 90 %~1 0 0 %。 结论 :SGA可用于肝硬化营养状况评价 。 Objectives: To assess nutritional parameters of patients with hepatic cirrhosis and provide some useful evidence for the physician. Methods: Subjective global assessment (SGA), anthropometry and biochemistry parameters were examined in 43 patients with liver cirrhosis. Results: There were significant differences in BMI,TSF,AMC,CHI and TRF( P <0.05~0.001), respectively, among well nourished, mild to moderate malnutrition and severe malnutrition patients according to SGA. The abnormal rates of ALB and TRF were 45.45% and 54.55% in well nourished group according to SGA, but it reached to 70% and 90% in severe malnutrition group. The abnormal rates of CHI were 81.82%, 90.91% and 100% in well nourished, mild to moderate and severe malnourished patients, respectively, as classified by SGA. Conclusions: SGA is effective standard in evaluating the nutritional status of liver cirrhosis. CHI and TRF can be applied to assess the nutritional status of liver cirrhosis and were more sensitive than ALB, TSF, AMC and BMI.
