摘要
目的:通过对人脑挫裂伤后脑组织中细胞凋亡的研究,了解人脑挫裂伤后脑组织中凋亡发生的情况及凋亡在颅脑损伤过程中所起的作用。方法:采用末端脱氧核苷酸转移酶介导的d-UTP生物素标记法(TUNEL法)检测细胞凋亡,同时显微镜观察病理切片。结果:66.7%出现TUNEL阳性,TUNEL阳性与阴性患者间的GCS评分(t=-2.88,P=0.01)、受伤时间(t=2.14,P=0.049)差异有显著性意义。TUNEL阳性与阴性患者间性别(χ2=0.26,P=0.61)、年龄(t=-1.06,P=0.30)、预后(χ2=0.26,P=0.61)差异无显著性意义。结论:人脑挫裂伤后脑组织中存在凋亡,并且与患者的病情严重程度及病程相关。其在颅脑损伤的病理过程中有一定作用,但只是颅脑损伤后复杂病理过程中的一种表现,即创伤后神经细胞死亡的一种方式,其在机体的病理变化过程中似有双重作用。
AIM:To comprehend the occurrence effects of apoptosis on cranial trauma throug h the investigation of apoptosis in brain tissue after cerebral contusion and la ceration in human. METHOD:Brain tissue was obtained during débridement and/or frontotemporal pol e incision and internal decompression from 21 patients with brain contusion/lace ration. Terminal dUTP nick end labeling (TUNEL) was used to examine the cell apo ptosis. Light microscope was used to observe the pathological section. RESULT:TUNEL showed that positive cell apoptosis was found in 14 of the 21 pat ients (66.7%). The cell apoptosis was correlated only to GCS score (t=-2.88, P =0.01), time after injury (t=2.14, P=0.049) but not to sex (-1.06,P=0.30) and prognosis (
