影响绵羊人工机械瓣置入存活实验的围术期因素

摘要 目的 通过人工机械瓣置入羊体内的实验研究 ,探讨建立慢性动物存活模型的围术期影响因素。方法  11只成年绵羊在体外循环并行下完成机械瓣置入手术 ,其中二尖瓣位置入人工二尖瓣 5例 ,肺动脉瓣置入人工主动脉瓣 6例。通过围术期的综合处理和严格科学的管理使动物长期存活。结果 手术死亡 1例 ,病死亡率为 9 1% ,死亡原因为低心排、停机困难。术后早期轻度呼吸道并发症 5例 ,其发生率为 45%。另外 ,术后一过性血尿 4例 ,尿道口感染 1例 ,张力性气胸 1例。存活的 10例羊生存时间均超过 3 0d ,并状况良好。结论 术前细致的准备 ,术中手术技术改进 ,术后早期严密的监护、积极预防术后感染和呼吸道并发症、并给予输血和营养支持 ,以及早期认真抗凝治疗 。 Objective The purpose of this study was identifying the early factor of successful animal model by implanted mechanical valve to the sheep for long term in vivo Methods Eleven adult sheep performed implanted mechanical valve under the cardio-pulmonary bypass with beating heart (or fibrillating heart) There were 5 cases with mitral prosthesis in mitral position and 6 cases with aortic prosthesis in pulmonary position The animal has been breaded for a long time after reasonable treatment in the perioperative period Results One sheep died in the operative period The cause of death was the low cardiac output syndrome There were 5 sheep with mild respiratory complication, 4 with hematuria, 1 with infection of urethral orifice and 1 with pneumothorax The survived period of the rest 10 sheep was all over 30 days with good condition Conclusion The incidence of successful chronic model would be improved by (a) careful preoperative preparation; (b) intraoperative technique modification; (c) intensive care, preventive infection and respiratory complication, blood transfusion, and nutritional support in the early postoperation; (d) early-term anticoagulation protocol
