冠心病患者血浆纤维蛋白原与小而密低密度脂蛋白的关系 被引量:21

Relationship between fibrinogen and small low-density loipoprotein particles in coronary heart disease

摘要 目的 探讨血浆纤维蛋白原 (Fib)水平升高和小而密低密度脂蛋白 (sLDL)在冠心病发病中的作用及两者的关系。方法 采用Clauss法测定 2 3 4例经冠状动脉造影证实为冠心病的患者及164例经冠状动脉造影证实无冠状动脉病变的对照者血浆Fib水平 ,以密度梯度聚丙烯酰胺凝胶电泳法测定sLDL在低密度脂蛋白中所占的比例 ,同时检测血清胆固醇 (TC)、甘油三酯 (TG)、低密度脂蛋白胆固醇 (LDL C)、高密度脂蛋白胆固醇 (HDL C)水平。根据冠状动脉造影的结果将冠心病组分为 1支、2支和 3支病变组 ,观察其与Fib及sLDL的关系。结果 冠心病组血浆Fib及血脂水平明显高于对照组 (HDL C低于对照组 ) ,P <0 0 5,Fib与sLDL水平随冠状动脉病变支数增加而升高 (P <0 0 5)。sLDL与冠状动脉病变程度密切相关 (r =0 452 ,P <0 0 0 1) ,与Fib水平呈正相关 (y =4 17x + 4 4 67)。结论 sLDL是冠心病发病的独立危险因素 ,Fib与sLDL呈正相关 。 Objective To investigate the relationship between small low-density lipoprotein (sLDL) particles and fibrinogen in coronary heart disease (CHD) Methods In a case-control study plasma lipids levels, small dense LDL percentage of total LDL and fibrinogen level were measured in 234 subjects with angiographically documented CHD and 164 subjects without CHD ( excluded by coronarography ) Fibrinogen level was detected by Clauss and sLDL level was detected by PAGE According angiography, patients were divided into single-, double- and triple- vessel groups We explored the relationship between small, dense LDL level and fibrinogen in different groups Results The levels of TC,TG, LDL, small LDL and fibrinogen were significantly higher in patients with CHD than those in controls (small LDL: 58 70±5 25% vs 54 67±6 03%, Fib: 3 30±0 79g/L vs 2 82±0 72g/L,P<0 05) The mean plasma HDL level was lower in CHD patients than that in the control group (P<0 05) As the stenosis vessels increased, the sLDL and fibrinogen levels also increased (P<0 05) The fibrinogen levels of no stenosis, single-, double- and triple-vessel group were 2 96±0 74 g/L, 3 32±0 90 g/L, 3 40±0 90 g/L, 3 52± 1 00 g/L The sLDL% was 53 79±5 90,59 47±5 51,58 77±3 82,61 08±5 69 in above groups respectively We found a strong positive correlation between small LDL percentage and coronary artery stenosis score (y=0 553 5x+54 693, r=0 452, P<0 001) and a positive correlation between sLDL and fibrinogen (y=4 17x+44 67) Conclusion Small dense LDL was an independent risk factor of CHD This study showed that fibrinogen was associated with the sLDL level and hyperfibrinogenemia may contribute to the increased CHD risk
