摘要
目的:回顾性评价晚期鼻咽癌调强放疗中双侧内耳道(internal auditory canal,IAC)的受量情况。方法:选择已完成调强放射治疗的13例鼻咽癌患者,所有患者均有明确病理诊断且均接受33次调强放疗。再由一名主治医师在原始的CT图像上勾画出双侧的内耳道,最后通过DVH图对双侧的内耳道受量进行分析,包括双侧内耳道的最大剂量(D_(max))、最小剂量(D_(min))及平均剂量(D_(mean))。结果:左侧内耳道的最大剂量(D_(max))、最小剂量(D_(min))及平均剂量(D_(mean))的均值分别为5 207.4c Gy、4 306.0c Gy、4 724.0c Gy;右侧内耳道的最大剂量(D_(max))、最小剂量(D_(min))及平均剂量(D_(mean))的均值分别为5 737.9c Gy、4 581.7c Gy、5 261.9c Gy。结论:晚期鼻咽癌的调强放疗中,在不牺牲肿瘤靶区的前提下建议进行内耳道勾画并限制其接受过高照射剂量。
Objective: To retrospectively evaluate the dose to the internal auditory canal( IAC) in patients with advanced nasopharyngeal carcinoma treated with intensity- modulated radiotherapy. Methods: Chose thirteen advanced nasopharyngeal carcinoma patients who have completed the intensity- modulated radiotherapy of 33 fractions. Then a attending physician contoured the IAC in the original CT images. Finally,we analysed the dose volume histogram( DVH) of the left and right IAC including the maximum dose( Dmax),minimum dose( Dmin) and the average dose( Dmean). Results: The mean of maximum dose of the left IAC( Dmax),minimum dose( Dmin) and the average dose( Dmean) were respectively 5 207. 4c Gy,4 306. 0 c Gy,4 724. 0 c Gy,and the right was 5 737. 9c Gy,4 581. 7c Gy,5 261. 9c Gy respectively. Conclusion: We suggest delineate and constrain the dose of IAC without sacrifice the tumor target region of the advanced nasopharyngeal carcinoma treated with intensity- modulated radiotherapy.
