解剖型胫骨弹性髓内钉与矩形髓内钉治疗胫骨干骨折比较 被引量:1

摘要 目的 比较解剖型胫骨弹性髓内钉 (AFRIN)与矩形髓内钉 (RSIN)治疗胫骨干骨折的性能差异。方法  5 6例胫骨骨折随机分为两组 ,分别采用AFRIN与RSIN固定 ,比较手术操作时间、插钉次数、手术出血量、术后平均骨折愈合时间、石膏固定时间、骨折对线情况等。结果 AFRIN组与RSIN组比较 :平均插钉时间分别为 (2 1± 4 )min和 (32± 5 )min ;平均插钉次数分别为 (1 6± 0 4 )次和 (2 8± 0 5 )次 ;平均出血量分别为 (2 5± 5 )ml和 (31± 4 )ml;平均骨折愈合时间分别为 (14 8±0 5 )周和 (15 1± 0 6 )周 ,平均石膏固定时间分别为 (16± 4 )d和 (2 5± 3)d。骨折愈合对线情况 :正位片成角AFRIN组 (1 8± 0 6 )° ,RSIN组 (3 5± 0 7)° ;侧位片成角AFRIN组 (1 6± 0 3)° ,RSIN组(2 7± 0 4 )°。结论 AFRIN治疗胫骨骨折比RSIN插钉更方便 ,手术效果更优。 Objective To investigate the outcome of tibial shaft fracture treated with anatomical flexible rectangle intramedullary nails(AFRIN) and rectangle shaped intramedullary nails(RSIN), respectively. Methods 56 cases of tibial shaft fractures were divided randomly into 2 groups of A and B. Group A were fixed with AFRIN and group B with RSIN. The operation time, times of inserting nails, blood loss,time of fracture healing, time of plaster fixation and the alinement of the shaft were analysised respectively. Results The average operation time is (21±4) min for AFRIN and (32±5) min for RSIN;the average times of inserting nails were (1 6±0 4) times for AFRIN and (2 8±0 5) times for RSIN,the average blood loss was (25±5) ml for AFRIN and (31±4) ml for RSIN, the average time of fracture healing was (14 8±0 5) weeks for AFRIN and (15 1±0 6) weeks for RSIN; and the time of plaster fixation was (16±4) d for AFRIN and (25±3) d for RSIN. The angulation in anterior posterior films was (1 8±0 6)° for AFRIN and (3 5±0 7)° for RSIN; in lateral films, it was (1 6±0 3)° for AFRIN and (2 7±0 4)° for RSIN. Conclusion It is more convenient to fix the tibial shaft fracture with AFRIN than with RSIN, and the outcome of AFRIN is better than that of RSIN.
