摘要
目的探讨长骨骨折内固定器材破坏的特点和原因。方法作者对1056例计1297个部位经内固定术治疗的四肢长骨骨折病例进行了回顾性分析。结果内固定器材破坏52个部位,占401%。内固定器材破坏具有如下特点:①多属疲劳性破坏;②破坏率与骨折愈合时间有关,与内固定器材种类无关;③好发于粉碎性骨折,尤其是复杂性粉碎骨折;④下肢破坏率明显高于上肢。结论复位后骨折端缺乏有效骨质支撑或内固定器材选择和使用有缺陷是导致破坏的内在原因;而术后管理不当是导致破坏的外在原因。
Objective To study the reason and characteristics of internal fixation apparatus destruction in long bone fractures.Methods An analysis of 1056 cases with long bone fractures was performed.Results Implant failure was found in 52 cases and the failure rate was 4 01 per cent.The characteristics of internal fixation apparatus destruction:(1)Fatigue destruction was often observed.(2)Destruction rate was relative to fracture healing time.(3)Destruction was often observed in comminuted fractures.(4)Failare rate of pectoral limb was more than that of pelvic limb.Conclusion Lack of the effective support between the fracture ends or an error of internal fixation apparatus selection and use was the internal destruction cause.Incorrect postoperative management was the external destruction cause.
