别隐品碱对家兔实验性心肌梗塞心功能及血流动力学的影响

Effects of Allocryptopine on Experimental Myocardial Infarction,Cardiac Function and Hemodynamics in Rabbits

摘要 结扎兔冠状动脉左室支,造成急性心肌梗塞(AMI)模型,观察别隐品碱(Allocryptopine、Ap)5mg/kg、Img/kg、4mg/kg 分别对 AMI 范围(MIE)、心电图、血清 CPK 和 LDH-同功酶水平,心功能、血流动力学、大鼠复灌心律失常进行了研究。结果表明,AP 显著缩小 MIE,明显减轻心电图型的缺血和坏死性变化,降低血清 CPK 及 LDH-同功酶水平.减慢心率(HR)、降低动脉收缩压(SBP)和舒张压(DBP)、左室内压(LVP)、抑制左室收缩与舒张功能、缩短室性心律失常持续时间和室颤发生数。提示:AP 能减轻缺血损伤程度,具有抗心肌梗塞的作用。 Allocryptopine 5 mg/kg iv decreased ST and ∑ST as well as Q in the precordial multiple ECG mapping after the left ventricular branch of coronary artery was ligated.And it diminished the extent of myocardial infarction(MIE)from 25.0±2.9%(control)to 14.4±2.7%(p<0.01).It was found that allocryptopine lmg/kg iv had a protective effect on rats with arrhythmia induced by liga- tion and reperfusion of their coronary arteries and decreased the frequency of arrhythmia and ventricu- lar fibrillation and death rate.Allocryptopine 5 mg/kg iv diminished SCPK and LDH-isoenzyme level. sCPK from 614.0±46.6 IU/I(infarcted control)to 406.1±21.2 IU/I(P<0.01),LDH-isoenzyme from 72.8%±4.16%(infarcted control)to 4 6.4%±11.09%(P<0.01).Allocryptopine 4mg/kg iv lowered systolic blood pressure(SBP),Diastolic blood pressure(DPB),Left ventricular systolic pressure(LVSP)and heart rate(HR),but increased left ventricular enddiastolic pressure(LVEDP). Also allocryptopine 4mg/kg iv significantly depressed the indices of the left ventricular contractile function:decreased maximal rate of change of intraventricular pressure(dp/dt max)and Vec at dp/ dt_max,and prolonged the time of zero to dp/dt max(t-dp/dt_max)as well as left ventricular ejection time (LVET).The drug also depressed the indices of the left ventri-cular diastolic function:reduced maxi- mal lowering rate of intrav-entricular pressure(—dp/dt_max),—Vce at —dp/dt_max,the ratio of —dp/ dt_max,to LVSP and The time of LVSP to halffT—P/2).The above results suggest that allocryptopine can improve myocardial ischemia and exert a beneficial effect on acute myocardial infarction.
