摘要
目的:观察序贯联合应用雌孕激素治疗围绝经期及绝经早期绝经相关症状和调整月经周期的疗效与阴道出血的状况。方法:多中心研究有绝经相关问题的围绝经期或绝经早期妇女,应用倍美盈雌孕激素复方制剂治疗3个周期,观察治疗前后Creene症状评分、阴道出血情况、副反应、子宫及其内膜。结果:接纳研究对象107例,完成99例。Creene总评分及其亚项(心理、躯体、血管舒缩和性功能障碍等症状)均显著降低,治疗前总分为17.88±12.78,治疗后为4.15±4.54(P<0.01);用药第1、2、3个周期规律出血发生率分别为64.6％、61.6％、59.6％;不规律出血发生率为20.2％、27.3％、26.3％;无出血周期率15.2％、11.1％、14.1％;乳胀发生率32.3％～44.4％。继续随诊到的68例中,42.6％停药,57.4％继续雌孕激素治疗。结论:序贯联合应用倍美盈,可有效地缓解绝经相关症状,阴道出血较规律。治疗中不需记忆加孕激素的时间,简便易行。
Objective: To observe the efficacy of Premelle cycle in managing the menopausal symptoms and vaginal bleeding patterns. Methods: Women during perimenopausal period were recruited in multiple centers. They received Premelle cycle 1 tablet/day for 28 days/month. The menopausal symptoms were evaluated by the Greene scores before treatment and the 1st, 2nd, 3rd month of treatment. The compliance, the acceptability and the side effect were observed at the end of the study. Results: 107 cases were recruited and 99 completed the trial. The menopausal symptoms were alleviated. The total score decreased from 17. 88 ± 12. 78 before treatment to 4. 15 ±4. 54 at the end of the trial (P <0. 01) ;The regular bleeding rate were 64. 6% (1st month) , 61.6% (2ndmonth) , 59. 6% (3rdmonth). The irregular bleeding rate were 20. 2% ,27. 3% ,26. 3%. The rate of breast swelling were 32. 3% -44. 4% , which decreased as the duration prolonged. The compliance was fine. Sixty - eight cases were followed up after the trial. 57. 4% continued the treatment and 42. 6% stopped. Conclusions:The Premelle cycle is recommended to prescribe to women during perimenopausal period to control the menstrual periods and release the menopausal symptoms. The irregular bleeding rate and side effects were low. Taking the Premelle cycle was easy and safe.
