摘要
目的 :比较两种剂量达必佳在IVF超促排卵降调节中的作用。方法 :将行IVF和ICSI治疗的58例不孕患者70个治疗周期 ,按其黄体期使用达必佳的剂量分成 :0.1mg/日组和0.05mg/日组。降调节后月经第三天测定血LH、E2 水平和子宫内膜厚度 ,超排过程中当主导卵泡直径≥14mm时 ,每日监测尿LH ,比较两组HCG日子宫内膜厚度、成熟卵泡数、取卵数、达必佳用量和促性腺激素用量。结果 :两组降调节后月经第三天血LH、E2 水平和子宫内膜厚度 ,HCG日子宫内膜厚度、成熟卵泡数、取卵数和促性腺激素用量均无显著性差异 ,而达必佳用量有显著性差异 ,0.05mg/日组少于0.1mg/日组 ,注射HCG前两组中无一例尿LH阳性。结论 :使用达必佳0.05mg/日降调节方案能有效地防止内源性LH峰的出现 ,达到降调节的效果 。
Objective:To compare the effects of two doses of triptorelin acetate on the down regulation of the controlled ovarian hyperstimulation in IVF.Methods:58patients with70cyˉcles were divided into two groups with different daily dose of triptorelin acetate:0.1mg/d or0.05mg/d injection started in the midluteal phase of the preceding cycle.The menstrual cycle day3serum concentration of LH,oestradiol and endometrial thickness were determined.Urine LH was monitored two times each day since the leading follicle(s)reached14mm.endometrial thickness and matured follicles on the day of HCG and the number of oocyte retrievaled were compared.The total doses of triptorelin acetate and gonadotropin(Gn)were caculated.Results:The serum concentration of LH,oestradiol and endometrial thickness on day3,endometrial thickness and matured follicles on the day of HCG,the number of oocyte retrievaled,total dose of Gn showed no significant difference between the two groups.The total dose of triptorelin acetate in daily0.05mg group was lower than daily0.1mg group.The urine LH testes were negtive in both two groups before human chorionic gonadotropin(HCG)injection.Conclusion:A daily dose of0.05mg triptorelin was sufficient to prevent an undesired premature LH surge during IVF treatment and the cost seem to be lower than the daily dose of0.1mg group.
