摘要
目的 探讨在HIFU治疗过程中应用控制性呼吸和人工胸水的临床安全性 ,以及对HIFU治疗的意义。方法 12例肝肿瘤患者在HIFU治疗过程中采用气管或双支气管插管法控制呼吸 ,根据HIFU治疗需要 ,通过暂时阻断呼吸 ,以消除肝脏运动对HIFU治疗的影响 ;其中对 5例病灶位于肝膈顶部的患者 ,同时在右侧胸膜腔内注入生理盐水 ,推开治疗入路的肺组织 ,显露并治疗肝膈顶部肿瘤。结果 控制性呼吸暂停后 ,肝脏随呼吸的运动明显减小或消失 ,使HIFU治疗能在肝脏静止状态下进行 ,注入人工胸水后 ,HIFU治疗入路的肺组织被推开 ,肝膈顶部清楚显露 ;HIFU治疗肝膈顶肿瘤顺利完成 ,所有治疗均无相关并发症发生。结论 此两项措施是安全的 ,有助于提高HIFU治疗的效率和安全 。
Objective To evaluate the efficacy and safety of clinical application of controllable respiration and artificial pleural fluid to aid HIFU treatment of liver carcinoma.Methods Twelve patients with liver carcinoma were received tracheal cannula to control respiration during HIFU treatment. In order to decrease the influence of liver movement during HIFU treatment, patients are controlled to stop respiration temporarily. Five patients of them whose tumor lesions located near the diaphragm were infused saline into right pleural cavity. The lung tissue was pushed away from the therapeutic passage to expose the tumor in the distal end of liver diaphragm.Results After using controllable respiration, the movement of the liver affected by lung movement decreased or disappeared. After infusing artificial pleural fluid, the lung tissue was pushed away from the therapeutic passage, the distal end of diaphragm appeared clearly. HIFU treatment can proceed smoothly. No patients were found with correlative complications.Conclusion The two measures are safe.They are helpful to improve efficacy of HIFU treatment and enlarge therapeutic ranges of HIFU treatment on liver carcinoma.
