摘要
本文研究了小剂量7.5%NaCl-6%Dextran70液对模拟4 000m高原条件下,失血性休克犬心血管功能和存活率的影响。经动脉放血,在8min内使MAP降至5.73±0.1kPa,并维持在此水平60min后,分别经静脉快速注入0.9%NaCl(NS组,n=7)、7.5%NaCl(HS组,n=6)和7.5%NaCl-6%Dextran 70(HSD组,n=6),剂量均为4ml/kg,在2min内注完。治疗120min后,NS组的存活率为2/7,而HS组和HSD组的存活率均为5/6。与NS相比,HS或HSD治疗均能显著升高平均动脉血压,增强心功能。其中HSD组治疗后各动力学指标升高幅度均大于HS组相应值,但两组间比较无显著的统计学差异。与NS组比较,HS组和HSD治疗后肺血管阻力呈下降趋势。治疗后HSD组的血浆容量扩充的百分量显著高于NS组(P<0.01)和HS组(P<0.05)。本实验结果表明,静脉快速注入小剂量HS或HSD对高原失血性休克均有显著的救治作用,其中HSD的作用略优于HS。
The effects of 7.5% NaCl-6% dextran 70 in small dosage on the cardiovascular functions and the survival rate in hemorrhagic shock at a simulated high altitude of 4 000 meters were observed in dogs.The animals were bled from the femoral artery for 8 minutes to reduce the mean arterial pressure to 5.73±0.1 kPa,which was maintained for an hour.Then the dogs were intravenous injected with 4 ml/kg of 0.9% Nad (NS group,n=7 ),7.5% Nad (HS group,n=6) and 7.5% NaCl-6% dextran 70 (HSD group,n=6) respectively.In the 2-hour-period after injection,5 out of the 7 dogs of NS group died but only 1 out of the 6 of both the HS and HSD groups died.Significant increase of mean arterial pressure,left ventricular pressure,±dp/dtmas of the left ventricle,cardiac output,cardiac index,and cardiac minute work index was observed in both the HS and HSD groups.Improvement of the cardiovascular functions was even more marked in HSD group than in HS group though the difference between the 2 groups was statistically insignificant.There was a tendency for the pulmonary vascular resistance to decrease in HS and HSD groups as compared with NS group.The percentage of plasma expansion of HSD group was larger than that of HS group ( P < 0.05 ) and of NS group ( P < 0.01 ),but no significant difference existed between HS and NS groups.It is believed that intravenous injection of hypertonic saline alone or in combination with dextran is effective for the treatment of hemorrhagic shock at high altitude and the combination of 7.5% Nad and 6% dextran 70 seems more effective.
