摘要
目的 :研究以头穴、体穴并用 ,阴阳经同取 ,健瘫侧交替方法 ,观察针刺治疗急性脑梗塞上肢运动功能障碍的疗效 ,并进行机理研究。方法 :选择脑梗死病程 5天内 ,具有上肢运动功能障碍者5 5例 ,随机分为观察组和对照组 ,对照组予以常规西药治疗 ;观察组在常规西药治疗基础上取患肢对侧头穴运动区中 2 / 5、感觉区中 2 / 5 ,健瘫侧肩骨禺、曲池、外关、合谷、极泉、尺泽、内关、大陵隔日交替针刺 ,加电针刺激 ,治疗 1个月。结果 :观察组 Fugl-Meyer积分高于对照组 ,说明其在改善上肢运动功能方面优于对照组。同时发现 ,此针刺疗法在降低内皮素、调节血栓素 A2 与前列环素 PGI2 平衡方面都优于对照组。结论 :针刺治疗能改善脑梗塞的病理环节 ,改善脑供血 ,保护脑细胞 。
Objective: To observe the curative effect and study its mechanism of acupuncture on cerebral infarction with upper extremity motor disfunction,by using points of scalp and body,yang and yin meridians with healthy side and paralytic side in turn.Methods:55 patients within a week course of cerebral infarction,whose upper extremity moved abnormally,were randomly devided into treatment group and control group.The control group was trenated by normal medicine.Addition to normal medicine,the treatment group was given acupuncture,on choosing points of opposite scalp (the middle 2/5 of movement zone and the middle 2/5 sensitive zone),points of Jianji,Jianyu,Quchi,Waiguan,Hegu,Jiquan,Chize,Neiguan,Dalinadding electro acupun cture,with the paraltic side and healthy side chosen in turn.Patients had been treated for one month.Results: The scores of Fugl Meyer upper extremity motor function in treaatment group was higher than that in control group,which showed that acupuncture improved upper extremity motor more than medicine did.Moreover,compared with the control group,the treatment group could significantly decrease the level of plasma endothelia,fibrinogen,and regulate the level of plasma TXA 2 PGI 2. Conclusion:The acupuncture method can improve cerebral infarction's pathology,provide more blood to brain and protect brain cells,so as to resolve the matter of cerebral infarction's upper extremity motor disfunction.
